摘要 目的探讨动态对比增强磁共振成像(DCE-MRI)和弥散加权成像(DWI)相关参数在术前评估脑星形细胞瘤恶性程度分级的价值。方法回顾性分析我院2019年10月~2021年10月经病理证实的48例脑星形细胞瘤患者,其中低级别星形细胞瘤20例,高级别星形细胞瘤28例。比较不同分级患者容积转运参数(K^(trans))、血管外细胞外间隙容积比(V_(e))、表观扩散系数(ADC)。通过ROC分析K^(trans)值、V_(e)、ADC预测高级别脑星形细胞瘤的价值。结果低级别脑星形细胞瘤患者K^(trans)值、V_(e)显著低于高级别脑星形细胞瘤患者,ADC显著高于高级别脑星形细胞瘤患者(P<0.05)。经过ROC分析,K^(trans)值、V_(e)、ADC预测高级别星形细胞瘤的曲线下面积分别为0.756、0.823、0.694(均P<0.05)。结论DCE-MRI联合DWI术前诊断脑星形细胞瘤相关参数和病变恶性程度相关,可为手术实施、治疗方案和预后评估提供参考。 Objective To explore the value of the relevant parameters of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)and diffusion-weighted imaging(DWI)in the preoperative diagnosis of astrocytoma.Methods The 48patients with astrocytomaadmitted to our hospital from October 2019to October 2021were selected as the study subjects.The patients were classified into low-grade astrocytic tumours(n=20)and high-grade astrocytic tumours(n=28).The volume transport parameters(K^(trans)),extravascular extracellular space volume ratio(V_(e)),apparent diffusion coefficient(ADC)in patients with different grades were measured and compared.The ROC curve was used to assess the value of K^(trans),V_(e),and ADC to predict high-grade astrocytoma.Results The K^(trans)values,V_(e)of patients with low-grade astrocytoma were significantly lower than those with high-grade astrocytoma,and ADC was significantly higher than that of patients with high-grade astrocytoma(P<0.05).After ROC analysis,the area under the curve of K^(trans),V_(e),and ADC predicting highgrade astrocytoma were 0.756、0.823、0.694(all P<0.05),respectively.Conclusion DCE-MRI and DWI preoperative diagnosis of astrocytoma parameters are related to malignant degree of astrocytoma patients,which can provide a reference for the implementation of surgery,treatment program and prognostic evaluation.
