摘要
目的:基于数据挖掘探讨化疗相关性恶心呕吐穴位贴敷的取穴用药规律。方法:以中国知网、万方、维普、中国生物医学文献数据库为检索库,检索自建库至2020年5月1日各库中有关穴位贴敷治疗化疗相关性恶心呕吐的文献,建立本研究数据库。运用SPSS 22.0进行聚类分析,运用SPSS Modeler 18.0中的Apriori算法进行关联规则分析。结果:共检索出符合要求的文献45篇,病例数2036例,共涉及40味药,总频次205,共涉及14个穴位,总频次121。常用穴位有内关、足三里、中脘、神阙。贴敷用药以生姜、半夏、吴茱萸为主。聚类分析结果有6组。药物关联规则分析共得到核心中药组合15种。结论:临床上运用穴位贴敷治疗化疗相关性恶心呕吐,常用辛温之性的药物,配合内关、足三里、中脘、神阙等穴位,从脾胃肺三个方面进行论治。
Objective:To explore the rule of acupoint application for chemotherapy-related nausea and vomiting based on data mining.Methods:CNKI,Wanfang,Weipu,and CBM were used as search databases,the literature on the treatment of chemotherapyrelated nausea and vomiting by acupoint application in each library from the time of establishment to May 1,2020 were searched.SPSS 22.0 was used for cluster analysis,SPSS Modeler 18 software and Apriori algorithm were used for association rules analysis,the law of compatibility of drugs and acupoints was explored.Results:A total of 45 eligible literatures were retrieved,including 2036 cases,involving 40 herbs with a total frequency of 205,involving 14 acupoints and a total frequency of 121.The commonly used acupoints in acupoint application are Neiguan(内关),Zusanli(足三里),Zhongwan(中脘),Shenque(神阙).The core medications for application are ginger,Pinellia ternata and Evodia rutaecarpa.The results of cluster analysis were 6 groups:Pinellia ternata,ginger,Evodia rutaecarpa;Atractylodes macrocephala,Codonopsis;Tangerine peel,Radix Aucklandiae;Raddle,Inula flower;Coptidis rhizoma,Perilla stem;Dried ginger,Cinnamon.The analysis of drug association rules focused on ginger,Pinellia ternata,Evodia rutaecarpa,Codonopsis,Atractylodes macrocephala and clove.Conclusion:In the clinical application of acupoint application in the treatment of chemotherapy-related nausea and vomiting,warm and acrid herbs combined with Neiguan,Zusanli,Zhongwan,Shenque and other acupoints are commonly used.The treatment is conducted from three aspects of spleen,stomach and lung.
