摘要
目的:探讨中药穴位贴敷联合布托啡诺、丙泊酚在无痛人工流产手术中的应用效果。方法:选择近年来在广州市中西医结合医院进行无痛人工流产手术的270例患者作为研究对象。随机将其分为Ⅰ组、Ⅱ组和Ⅲ组。术中用丙泊酚对Ⅰ组患者进行麻醉,Ⅱ组患者在丙泊酚麻醉的基础上静脉注射布托啡诺,Ⅲ组患者在丙泊酚麻醉及静脉注射布托啡诺的基础上进行中药穴位贴敷,然后比较三组患者的各项临床指标。结果:术毕,Ⅰ组患者的平均动脉压(MAP)低于Ⅱ组、Ⅲ组患者,其心率(HR)高于Ⅱ组、Ⅲ组患者,P<0.05;Ⅱ组、Ⅲ组患者的MAP、HR相比,P>0.05。Ⅰ组患者丙泊酚的用量大于Ⅱ组、Ⅲ组患者,其麻醉持续的时间长于Ⅱ组、Ⅲ组患者,P<0.05。Ⅱ组、Ⅲ组患者丙泊酚的用量、麻醉持续的时间相比,P>0.05。三组患者手术的时间相比,P>0.05。术后24 h,Ⅰ组患者的疼痛分级和疼痛的发生率均高于Ⅱ组、Ⅲ组患者,P<0.05;Ⅱ组患者疼痛的分级和疼痛的发生率均高于Ⅲ组患者,P<0.05;Ⅰ组、Ⅱ组患者的恶心呕吐分级和恶心呕吐的发生率均高于Ⅲ组患者,P<0.05。结论:将中药穴位贴敷联合布托啡诺、丙泊酚应用于无痛人工流产手术中,能显著提高患者术中、术后的镇痛效果,降低其术后恶心呕吐的发生率。
Objective:To explorethe effect of acupoint application of traditional Chinese medicine combined with butorphanol and propofol for the patients with painless abortion surgery.Methods:A total of 270 patients who underwent painless abortion in Guangzhou Hospital of Integrated Traditional Chinese and Western Medicine from March 2020 to June 2020 were selected as the research objects,and they were divided into 3 groups using a number random table method,with 90 cases in each group.Group I was anesthetized with propofol,groupⅡwas given intravenous butorphanol on the basis of propofol anesthesia,and groupⅢwas given acupoint application of traditional Chinese medicine combined with intravenous butorphanol on the basis of propofol anesthesia.Demographic,anesthesia,and surgical characteristics of each group of patients,pain,nausea and vomiting 24 hours after the operation were collected.The differences of the above indicators among the three groups were analyzed and compared.Results:The preoperative MAP,HR,and SpO2 of the three groups were compared,and the difference was not statistically significant(P>0.05).In groupⅠ,the postoperative MAP was lower than that in groupsⅡandⅢ,the postoperative HR was higher than in groupsⅡandⅢ,the dosage of propofol was greater than in groupsⅡandⅢ,and the anesthesia time was longer than that in groupsⅡandⅢ,the differences were statistically significant(P<0.05).There was no statistically significant difference in postoperative SpO2 and operation time among the three groups(P>0.05).The grade and incidence of postoperative pain in groupⅠwere significantly higher than those in groupsⅡandⅢ,and groupⅡwas significantly higher than groupⅢ(P<0.05).The classification and incidence of pos toperative nausea and vomiting in groupⅠandⅡwere significantly higher than those in groupⅢ(P<0.05).Conclusion:Acupoint application of traditional Chinese medicine combined with butorphanol and propofol can significantly enhance the analgesic effect of painless abortion during and
