高强度间歇性呼吸肌训练对脑卒中患者肺功能及活动能力康复效果的研究 被引量:9

摘要 目的观察高强度间歇性呼吸肌训练对脑卒中患者肺功能及活动能力的康复效果,以及呼吸肌肌力变化与运动功能、活动能力的相关关系。方法选取2021年1—10月新疆医科大学第一附属医院康复医学科收治的脑卒中患者50例,采用随机数字表随机分为对照组、试验组,每组25例,对照组给予三球呼吸训练,试验组给予高强度间歇性呼吸肌训练,2组患者每周训练5 d连续训练14 d。分别比较两组患者治疗前后最大吸气压力(Maximum inspiratory pressure,MIP)、最大呼气压力(Maximum expiratory pressure,MEP)、呼气峰流速(Peak expiratory flow,PEF)、Fugl-Meyer运动功能评分(Fugl-Meyer assessment scale,FMA)、莫顿活动指数(The de morton mobility index,DEMMI)和改良巴氏指数评定量表(MBI)。结果治疗2周后,与本组治疗前比较,治疗后患者MIP、MEP、PEF、FMA、DEMMI、MBI指标均升高,差异有统计学意义(P<0.05)。与对照组比较,试验组患者治疗后MIP、MEP、PEF、FMA、DEMMI、MBI指标升高,差异有统计学意义(P<0.05)。MIP的提高与DEMMI、MBI的提高呈正相关关系。结论高强度间歇性呼吸肌训练可有效改善脑卒中患者肺功能及活动能力,且吸气肌力的提高可明显改善莫顿活动指数及日常生活活动能力。 Objective To observe the rehabilitation effect of high-intensity interval respiratory muscle training on pul⁃monary function and activity ability of stroke patients,and the relationship between respiratory muscle strength and motor function and activity ability.Methods A total of 50 stroke patients admitted to the hospital from January 2021 to October 2021 were selected.25 patients in each group were randomly divided into the control group and the experi⁃mental group by random number table.The control group was given three-ball breathing training and the experimental group was given high-intensity intermittent breathing muscle training.The differences of indexes before and after the treatment in the two groups of patients were compared,including maximum inspiratory pressure(MIP),maximum expi⁃ratory pressure(MEP),peak expiratory flow(PEF),Fugl Meyer rating scale(FMA),de Morton activity index(DEMMI)and modified Barthel index(MBI).Results After 2 weeks of the treatment,compared with before the treatment,the in⁃dexes of MIP,MEP,PEF,FMA,DEMMI and MBI were increased after the treatment,with statistical significance(P<0.05).Compared with the control group,the indexes of MIP,MEP,PEF,FMA,DEMMI and MBI in experimental group were increased after the treatment,and the differences were statistically significant(P<0.05).The improvement of MIP was positively correlated with improvement of DEMMI and MBI.Conclusion High-intensity interval respiratory mus⁃cle training can effectively improve pulmonary function and activity in stroke patients,and improvement of MIP can significantly improve MBI and activity of daily living(ADL).
