摘要
目的分析近5年大肠埃希菌及肺炎克雷伯菌的耐药率与抗菌药物使用强度(AUD)的相关性,为临床合理使用抗菌药物提供参考。方法回顾性分析我院2016~2020年大肠埃希菌及肺炎克雷伯菌的耐药率与同期主要AUD,采用Pearson相关性分析法分析两者的相关性。结果大肠埃希菌的耐药率有所下降,而肺炎克雷伯菌的耐药率明显上升。大肠埃希菌对左氧氟沙星、头孢呋辛的耐药率分别与其AUD呈正相关(P<0.05),对头孢他啶的耐药率与头孢他啶、头孢哌酮舒巴坦的AUD呈正相关(P<0.01,P<0.05),对阿米卡星的耐药率与美罗培南、哌拉西林/他唑巴坦和亚胺培南的AUD呈负相关(P<0.05),对头孢哌酮/舒巴坦的耐药率与头孢他啶的AUD呈负相关(P<0.05)。肺炎克雷伯菌对左氧氟沙星、头孢他啶的耐药率与头孢呋辛的AUD呈负相关(P<0.05,P<0.01),对头孢呋辛的耐药率与头孢西丁、头孢哌酮/舒巴坦的AUD呈负相关(P<0.05,P<0.01),对头孢噻肟的耐药率与其AUD呈正相关(P<0.05)。结论大肠埃希菌及肺炎克雷伯菌的耐药率与多种抗菌药物的AUD相关,应加强抗菌药物的合理管控,延缓细菌耐药。
Objective To analyze the correlation between drug resistance rate of Escherichia coli and Klebsiella pneumoniae and antibacterial use density(AUD)in recent 5 years,so as to provide reference for rational use of antibiotics in clinical practice.Methods The drug resistance rates of Escherichia coli and Klebsiella pneumoniae in our hospital from 2016 to 2020 and AUD of major antibiotics in the same period were retrospectively analyzed,and the correlation was analyzed by Pearson correlation analysis.Results The drug resistance rate of Escherichia coli decreased while that of Klebsiella pneumoniae increased significantly.The drug resistance rates of Escherichia coli to levofloxacin and cefuroxime were positively correlated with their AUD(P<0.05),respectively;the drug resistance rate to ceftazidme were positively correlated with AUD of ceftazidme,cefoperazone/sulbactam(P<0.01,P<0.05);the drug resistance rate to amikacin was negatively correlated with AUD of meropenem,piperacillin/tazobactam and imipenem(P<0.05);the resistance rate to cefoperazone/sulbactam was negatively correlated with AUD of ceftazidme(P<0.05).The drug resistance rate of Klebsiella pneumoniae to levofloxacin,ceftazidme was negatively correlated with AUD of cefuroxime(P<0.05,P<0.01),the resistance rate to cefuroxime was negatively correlated with AUD of cefoxitin,cefoperazone/sulbactam(P<0.05,P<0.01),the drug resistance rate of cefotaxime was positively correlated with its AUD(P<0.05).Conclusion The drug resistance rates of Escherichia coli and Klebsiella pneumoniae are related to AUD of a variety of antimicrobial drugs,and reasonable control of antimicrobial drugs should be strengthened to delay bacterial resistance.
