摘要
目的探究肾康丸对糖尿病肾病(diabetic nephropathy,DN)大鼠肾功能、胰岛素抵抗和足细胞的影响。方法高糖饮食联合腹腔注射链脲佐菌素(streptozotocin,STZ)制备糖尿病肾病大鼠模型。将造模成功的大鼠分为糖尿病肾病模型组(DN模型组)、肾康丸治疗组,另设正常组作为对照。灌服肾康丸3周后,检测大鼠左肾质量、血清肌酐、尿素氮、胰岛素、血糖、总胆固醇(total cholesterol,TC)、三酰甘油(triglyceride,TG)和微量尿蛋白,并计算胰岛素抵抗指数;用荧光定量PCR和蛋白印迹法检测大鼠肾小球足细胞裂隙跨膜基因(nephrin)、肌间线蛋白(desmin)mRNA和蛋白的表达情况,用透射电镜观察足细胞超微结构的变化。结果与正常组相比,DN模型组大鼠左肾质量/体质量、血肌酐、尿素氮、血糖、胰岛素抵抗指数、TG、TC、微量尿蛋白、desmin蛋白和mRNA的表达水平显著上升,胰岛素含量、nephrin蛋白和mRNA的表达水平显著降低(P<0.05);与DN模型组相比,肾康丸治疗组血肌酐、尿素氮、血糖、胰岛素抵抗指数、TG、TC、微量尿蛋白、desmin蛋白和mRNA的表达水平显著降低,胰岛素含量、nephrin蛋白和mRNA的表达水平显著升高(P<0.05),但肾康丸治疗组与DN模型组左肾质量/体质量的比较,差异无统计学意义(P>0.05)。足细胞透射电镜图显示,与DN模型组比较,肾康丸治疗组上述肾小球基底膜增厚、足突排列不齐、融合和数量减少等现象明显减轻。结论肾康丸可缓解胰岛素抵抗,改善肾足细胞的超微结构,减轻足细胞的减少与融合,发挥保护肾功能的作用。
Objective To investigate the effect of Shenkang Pills on renal function,insulin resistance and podocyte in rats with diabetic nephropathy(DN).Methods The DN rat models were prepared by high-glucose diet combined with intraperitoneal injection of streptozotocin(STZ).And they were divided into diabetic nephropathy model group(DN model group)and Shenkang Pills treatment group.And also normal group was established.After Shenkang Pills administration for 3 weeks,the weight of left kidney,serum creatinine,blood urea nitrogen,insulin,blood glucose,total cholesterol(TC),triglyceride(TG)and microalbuminuria were detected.The insulin resistance index was calculated.The mRNA and proteins of transmembrane genes nephrin and desmin in glomerular podocytes fissure were detected by fluorescence quantitative PCR and Western blot.The changes in the ultrastructure of podocytes were observed by transmission electron microscope.Results Compared with normal group,the weight/body mass of left kidney,serum creatinine,blood urea nitrogen,blood glucose,insulin resistance index,TG,TC,microalbuminuria,protein and mRNA expressions of desmin were significantly increased in DN model group,while the insulin content,protein and mRNA expressions of nephrin were significantly decreased(P<0.05).Compared with DN model group,the serum creatinine,blood urea nitrogen,blood glucose,insulin resistance index,TG,TC,microalbuminuria,protein and mRNA expressions of desmin were significantly decreased in Shenkang Pills treatment group,while the insulin content,protein and mRNA expressions of nephrin were significantly increased(P<0.05).However,there was no significant difference in weight/body mass of left kidney between Shenkang Pills treatment group and DN model group(P>0.05).The images of transmission electron microscope for podocytes showed that compared with DN model group,the situations such as the abovementioned glomerular basement membrane thickening,uneven arrangement of foot process,fusion and reduction of number in Shenkang Pills treatment group we
