摘要
目的:探讨吞咽-摄食管理联合吞咽操训练对改善脑卒中后吞咽障碍患者功能的影响。方法:选取2020年6月-2021年3月莆田市第一医院收治的70例脑卒中后吞咽障碍患者作为研究对象,采用随机数字表法将其分为对照组(常规康复护理)和观察组(吞咽-摄食管理联合吞咽操训练),每组35例。比较两组临床效果、护理前后吞咽障碍筛查量表评分(Burke评分)、电视透视吞咽功能检查(videofluoroscpic swallowing study,VFSS)评分及生活质量评分。结果:观察组总有效率明显高于对照组(χ^(2)=11.67,P=0.00)。护理后,两组Burke评分均低于护理前,VFSS评分、各维度SWAL-QOL评分均高于护理前,且观察组均优于对照组,差异均有统计学意义(P<0.05)。结论:吞咽-摄食管理联合吞咽操训练,能有效促进脑卒中后吞咽障碍患者吞咽功能恢复,提高其生活质量,对患者预后具有积极作用,值得推广应用。
Objective:To explore the effect of swallowing-feeding management combined with swallowing exercise training on improving the function of patients with dysphagia after stroke.Method:A total of 70 patients with dysphagia after storke admitted to First Hospital of Putian City from June 2020 to March 2021 were selected as the research objects.They were divided into the control group(conventional rehabilitation nursing)and the observation group(swallowing-feeding management combined swallowing exercise training)by random number table method,35 cases in each group.The clinical effect,before and after nursing dysphagia screening scale(Burke score),videofluoroscpic swallowing study(VFSS)score and quality of life score of two groups were compared.Result:The total effective rate of the observation group was significantly higher than that of the control group(χ^(2)=11.67,P=0.00).After nursing,Burke scores of two group were lower than those before nursing,VFSS scores and all dimensions of SWAL-QOL scores were higher than those before nursing,and observation group were better than those of control group,the differences were statistically significant(P<0.05).Conclusion:Swallowing-feeding management combined with swallowing exercise training can effectively promote promote the recovery of swallowing function and quality of life in patients with dysphagia after stroke,it has a positive effect on prognosis,and it is worthy of promotion and application.
