摘要
目的:探究超声引导泡沫硬化联合腔内射频闭合术治疗临床-病因-解剖-病理生理分级(CEAP)4级下肢静脉曲张的效果及对疼痛应激反应、血流动力学的影响。方法:随机将辽宁省健康产业集团阜新矿总医院于2019年12月-2020年12月收治的82例CEAP 4级下肢静脉曲张患者分为参照组(n=41)和病例组(n=41)。参照组行大隐静脉高位结扎术联合剔剥术治疗,病例组行超声引导泡沫硬化联合腔内射频闭合术治疗,对比两组治疗效果、疼痛应激反应、血流动力学。结果:病例组治疗总有效率为95.12%,高于参照组的78.05%(P<0.05);病例组术后12 h视觉模拟评分法(VAS)评分、缓激肽(BK)、P物质(SP)水平均低于参照组(P<0.05);术后12 h,病例组平均动脉压(MAP)、心率均较参照组低,血氧饱和度(SpO_(2))较参照组高(P<0.05)。结论:CEAP 4级下肢静脉曲张患者采用超声引导泡沫硬化联合腔内射频闭合术治疗可改善症状,减轻疼痛应激,且对血流动力学影响较小,具有临床推广实效性。
Objective:To explore the effect of ultrasound guided foam sclerotherapy combined with intracavity radiofrequency closure on CEAP grade 4 varicose veins of lower extremities and its effects on pain stress response and hemodynamics.Method:A total of 82 patients with CEAP grade 4 varicose veins admitted to Fuxin Mine General Hospital of Liaoning Health Industry Group from December 2019 to December 2020 were randomly divided into reference group (n=41) and case group (n=41).The reference group was treated with high saphenous vein ligation combined with stripping.The case group was treated with ultrasound guided foam sclerotherapy combined with intracavity radiofrequency closure.The therapeutic effects,pain stress response and hemodynamics of the two groups were compared.Result:The total effective rate of the case group was 95.12%,which was higher than 78.05% of the reference group (P<0.05);12 h after operation,VAS score,BK and SP levels of case group were lower than those of reference group (P<0.05);12 h after operation,MAP and heart rate of the case group were lower than those of the reference group,SpO_(2)was higher than that of the reference group (P<0.05).Conclusion:Ultrasound guided foam sclerotherapy combined with intracavity radiofrequency closure can improve symptoms,relieve pain stress response,and reduce hemodynamic effects in patients with CEAP grade 4 lower extremity varicose veins.
