摘要
目的比较microRNA-21在结肠癌患者及健康对照血清中的表达水平,从而评价其在结肠癌诊断中的作用。方法回顾性选取2015年1月至2019年12月济宁医学院附属医院肿瘤科收治的60例结肠癌患者为结肠癌病例组和60名健康体检人群为对照组。采用荧光定量聚合酶链式反应的方法扩增检测两组人群血清microRNA-21的表达水平,统计microRNA-21在两组人群的表达水平差异;分析microRNA-21与病理学指标之间的关系;通过受试者工作特征曲线分析microRNA-21诊断结肠癌的价值。结果结肠癌病例组患者血清中microRNA-21为3.48±1.90,明显高于健康对照组(2.14±1.29),差异有统计学意义(P<0.001)。肿瘤分期T越晚,microRNA-21表达水平越高,T3期及T4期患者血清microRNA-21表达水平明显高于T1期和T2期患者,(4.02±2.13 vs.2.97±1.54,P=0.035)。合并淋巴结转移的结肠患者血清microRNA-21的表达高于无淋巴结转移的患者(4.05±1.41 vs.3.01±2.14,P=0.033)。microRNA-21的表达与患者性别、年龄、肿瘤分化程度、有无远处转移无关(P>0.05)。microRNA-21诊断结肠癌的敏感度为71.7%,特异度为70.0%,曲线下面积为0.714。结论microRNA-21在结肠癌患者血清呈高表达,对结肠癌诊断有一定价值。
Objective To explore the diagnostic value of serum microRNA-21 in patients with colon cancer,and to analyze its clinical significance.Methods Sixty cases of colon cancer patients and 60 healthy individuals from Affiliated Hospital of Jining Medical University were enrolled.Serum microRNA was extracted,and then the expression of microRNA-21 was detected by reverse transcription polymerase chain reaction(RT-PCR).The association between the expression of microRNA-21 and clinicopathological characteristics was analyzed for patients with colon cancer.Receiver operating characteristic curve analysis was used to calculate the sensitivity and specificity of serum microRNA-21.Results The expression level of serum microRNA-21 in patients with colon was higher than that of healthy individuals(3.48±1.90 vs.2.14±1.29,P<0.001).The expression of serum microRNA-21 in tumor stage of T 3/T 4 was significantly higher than those of T 1/T 2 stage(4.02±2.13 vs.2.97±1.54,P=0.035),and those with lymph node metastasis significantly higher than those without lymph node metastasis(4.05±1.41 vs.3.01±2.14,P=0.033).No statistical difference was found between the expression of microRNA-21 and age,sex,differentiation,or long distance metastasis(P>0.05).The expression of microRNA-21 were capable of separating colon cancer and healthy individuals,with the sensitivity and specificity of 71.7%and 70.0%,respectively,area under the curve was 0.714.Conclusion The results of this study indicated that microRNA-21 expression levels in serum can be considered as a novel non-invasive biomarker for the detection of colon cancer patients.
