摘要
目的 探讨高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)感染合并宫颈上皮内瘤变1级(cervical intraepithelial neoplasia 1,CIN 1)患者采用聚焦超声治疗后HR-HPV清除情况,分析HR-HPV清除相关影响因素。方法 回顾性分析2017年1月至2019年12月于川北医学院附属医院妇科门诊确诊为HR-HPV感染合并CIN 1并行聚焦超声治疗患者300例,随访患者术后HR-HPV转阴情况。结果 聚焦超声治疗后6个月,HR-HPV累积清除率为41. 48%,治疗后12个月累积清除率为71. 26%,治疗后24个月累积清除率为88. 81%。HR-HPV持续感染与非持续感染患者比较、年龄30岁及以下与30岁以上患者比较、有HPV16或18型感染与无HPV16或18型感染患者比较,HR-HPV清除率差异均无统计学意义(P> 0. 05);HR-HPV单型感染与多型感染患者比较,HR-HPV清除率差异有统计学意义(P <0. 05)。结论 聚焦超声治疗后能达到较满意的HR-HPV清除率,HR-HPV单型感染比多型感染更易清除。
Objective To explore the clearance of high-risk human papillomavirus( HR-HPV) in patients with HR-HPV infection combined with cervical intraepithelial neoplasia 1( CIN 1) treated with focused ultrasound,and analyze the factors related to HR-HPV clearance. Methods A retrospective cohort study was used to enroll 300 patients who diagnosed with HR-HPV infection combined with CIN 1 and treated with focused ultrasound in the Gynecological Clinic of the Affiliated Hospital of North Sichuan Medical College from January 2017 to December 2019,and followed up the postoperative HR-HPV negative status. Result 6 months after focused ultrasound treatment,the cumulative clearance rate of HR-HPV was 41. 48%,the cumulative clearance rate for 12 months after treatment was 71. 26%,and the cumulative clearance rate for 24 months after treatment was 88. 81%. Comparison of patients with persistent HR-HPV infection and with non-persistent infections,comparison patients aged 30 years and below and patients over 30 years old,comparison patients with HPV 16 or 18 infection and without HPV 16 or 18 infection,there were no significant differences in HRHPV clearance rate( P > 0. 05);HR-HPV clearance rate was significantly different between patients with single HR-HPV infection and patients with multiple HR-HPV infection( P < 0. 05). Conclusion Focused ultrasound treatment can achieve satisfactory HR-HPV clearance rate. HR-HPV single infection is easier to clear than multiple infections.
