摘要
目的:比较显微镜下经侧裂-岛叶入路与经颞叶皮质入路血肿清除术治疗高血压性基底节区脑出血患者的效果。方法:回顾性分析2018年3月至2020年3月该院收治的62例高血压性基底节区脑出血患者的临床资料,根据手术方法不同将患者分为观察组和对照组各31例。对照组采用经颞叶皮质入路血肿清除术,观察组采用显微镜下经侧裂-岛叶入路血肿清除术。比较两组围术期相关指标[手术时间、术中出血量、术后意识恢复时间、住院时间以及48 h血肿清除率]水平、格拉斯哥昏迷量表(GCS)评分、日常生活能力评分量表(ADL)评分及术后并发症发生情况。结果:观察组手术时间、术后意识恢复时间和住院时间短于对照组,48 h血肿清除率高于对照组,差异均有统计学意义(P<0.05);两组术中出血量比较,差异无统计学意义(P>0.05);观察组术后1周的GCS评分及术后2周的ADL评分均高于对照组,差异有统计学意义(P<0.05);两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:与经颞叶皮质入路相比,采用显微镜下经侧裂-岛叶入路血肿清除术治疗高血压性基底节区脑出血患者能够缩短手术时间、术后患者意识恢复时间和住院时间,提高血肿清除率、术后GCS评分和ADL评分,安全性与之相当。
Objective:To compare effects of microscopic hematoma removal through transsylvian-insula approach and transtemporal cortical approach on patients with hypertensive basal ganglia intracerebral hemorrhage.Methods:The clinical data of 62 patients with hypertensive basal ganglia intracerebral hemorrhage admitted to our hospital from March 2018 to March 2020 were retrospectively analyzed.According to different surgical methods,these patients were divided into observation group and control group,31 cases in each.The control group was treated with hematoma removal through the transtemporal cortex approach,while the observation group was treated with the microscopic hematoma removal through the transsylvian-insula approach.The perioperative related index levels[operation time,intraoperative blood loss,postoperative consciousness recovery time,hospitalization time,and 48 hour hematoma clearance rate],the Glasgow coma scale(GCS)score,the daily living ability(ADL)score and the postoperative complications were observed in the two groups.Results:The operation time,the postoperative consciousness recovery time and the hospitalization time in the observation group were shorter than those in the control group;the 48-hour hematoma clearance rate was higher than that in the control group;and the differences were statistically significant(P<0.05).There was no significant difference in the intraoperative blood loss between the two groups(P>0.05).The GCS score 1 week after the surgery and the ADL score 2 weeks after the surgery in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusions:Compared with the transtemporal cortical approach,the microscopic hematoma removal through transsylvian-insula approach in the treatment of hypertensive basal ganglia intracerebral hemorrhage can shorten the operation time,the postoperative recovery ti
