磁共振DTI表观扩散系数、各向异性分数与腰骶丛脊神经根受压的关系 被引量:3

Relationship between DTI apparent diffusion coefficient,fractional anisotropy score and compression of lumbosacral plexus spinal nerve roots

摘要 目的分析磁共振(magnetic resonance,MR)弥散张量成像(diffusion tensor imaging,DTI)的表观扩散系数(apparent diffusion coefficient,ADC)、各向异性分数(fractional anisotropy,FA)与腰骶丛脊神经根受压的关系。方法选择2019年7月~2021年5月本院收治的40例腰骶丛脊神经根受压患者设为研究组,随机选择40例体检健康者设为对照组,均进行MR DTI检查。比较两组L_(4)-S_(1)神经根的ADC值和FA值,研究组患侧与健侧神经根的ADC值和FA值,不同Lindblon分级的ADC值和FA值。结果对照组L_(4)-S_(1)的ADC值均显著低于研究组,L_(4)-S_(1)的FA值均显著高于研究组,差异均有统计学意义(P<0.05)。研究组患侧的L_(4)-S_(1)ADC值均显著高于健侧,L_(4)-S_(1)FA值均显著低于健侧,差异均有统计学意义(P<0.05)。研究组中,Lindblon 1级12例,2级19例,3级9例。1级患者的L_(4)-L_(5)ADC值显著低于3级患者,S_(1) ADC值显著低于2级和3级患者,2级患者的L_(4)-S_(1)ADC值显著低于3级患者;1级患者的L_(4)-S_(1)FA值显著高于2级和3级患者,2级患者的L_(4)-S_(1)FA值显著高于3级患者,差异均有统计学意义(P<0.05);1级和2级患者的L_(4)-L_(5)ADC值比较,差异均无统计学意义(P>0.05)。经Pearson相关性分析,ADC值与腰骶丛脊神经根受压呈正相关(P<0.05),FA值与腰骶丛脊神经根受压呈负相关(P<0.05)。结论随着腰骶丛脊神经根受压程度的加重,ADC值逐渐升高,FA值逐渐下降。 Objective To analyze the relationship between magnetic resonance(MR)diffusion tensor imaging(DTI)apparent diffusion coefficient(ADC),fractional anisotropy(FA)and spinal nerve root compression of the lumbosacral plexus.Methods Forty patients with lumbosacral plexus spinal nerve root compression from July 2019 to May 2021 were selected as the study group,and 40 healthy people with physical examination were selected as the control group,all of whom were examined by MR DTI.The ADC and FA values of the two groups of L_(4)-S_(1)nerve roots,the ADC and FA values of the affected and uninfected nerve roots in the study group,and the ADC and FA values of different Lindblon grades were compared.Results The ADC value of L_(4)-S_(1)in the control group was significantly lower than that in the study group,and the FA value of L_(4)-S_(1)was significantly higher than that in the study group(P<0.05).In the study group,the L_(4)-S_(1)ADC value of the affected side was significantly higher than that of the healthy side,and the L_(4)-S_(1)FA value was significantly lower than that of the healthy side(P<0.05).In the study group,there were 12 cases of Lindblon grade 1,19 cases of grade 2 and 9 cases of grade 3.The L_(4)-L_(5) ADC value of grade 1 patients was significantly lower than that of grade 3 patients,the S_(1) ADC value was significantly lower than that of grade 2 and 3 patients,and the L_(4)-S_(1)ADC value of grade 2 patients was significantly lower than that of grade 3 patients.The L_(4)-S_(1)FA value of grade 1 patients was significantly higher than that of grade 2 and 3 patients,and the L_(4)-S_(1)FA value of grade 2 patients was significantly higher than that of grade 3 patients(P<0.05).There was no significant difference in L_(4)-L_(5) ADC values between grade 1 and grade 2 patients(P>0.05).Pearson correlation analysis showed that ADC value was positively correlated with lumbosacral plexus spinal nerve root compression(P<0.05),and FA value was negatively correlated with lumbosacral plexus spinal nerve root compression(P<
