扩散张量成像ADC及eADC值与腰椎间盘退行性变的相关性研究 被引量:1

摘要 目的探讨扩散张量成像(DTI)表观扩散系数(ADC)及指数化表观扩散系数(eADC)与腰椎间盘退行性变的相关性。方法选取100例腰腿疼痛患者行磁共振T2 IDEAL及DTI检查,利用T2 IDEAL水相位对腰椎间盘进行Pfirrmann分级,利用DTI图测量各腰椎间盘髓核的ADC及eADC值,对比腰椎间盘各Pfirrmann分级的ADC及eADC值的差异性,并分析ADC及eADC值与Pfirrmann分级的相关性。结果腰椎间盘各Pfirrmann分级的ADC及eADC值总体差异均有统计学意义(P<0.05)。组间多重比较采用LSD法,结果显示ADC值除Ⅳ级与Ⅴ级间差异无统计学意义(P>0.05)外,其余各级间比较均有统计学意义(P<0.05)。eADC值组间比较,除Ⅰ级与Ⅱ级差异无统计学意义(P>0.05)外,其余各级间比较均有统计学意义(P<0.05)。相关性分析显示,ADC值与Pfirrmann分级呈负相关(r=-0.752,P<0.05),eADC与Pfirrmann分级呈正相关(r=0.736,P<0.05)。结论 ADC、eADC值与Pfirrmann分级呈高度相关性,腰椎间盘ADC及eADC值改变可准确反映腰椎间盘退行性变的程度。 Objective To investigate the correlation between apparent diffusion coefficient(ADC) and exponential apparent diffusion coefficient(eADC) of diffusion tensor imaging(DTI) and lumbar disc degeneration. Methods A total of 100 patients with lumbar and leg pain were selected for T2 IDEAL and DTI examination. Pfirrmann classification of lumbar intervertebral disc was performed by using T2 IDEAL water phase, the ADC and eADC values of the lumbar disc nucleus were measured by DTI map, difference between ADC and eADC values of each Pfirrmann grade of lumbar disc were compared, and correlation between ADC and eADC values and Pfirrmann grade was analyzed. Results There were statistically significant differences in ADC and eADC value among each Pfirrmann grading of lumbar intervertebral discs(P<0.05). LSD method was used for multiple comparisons between groups, and the results showed that there was no statistically significant difference in ADC values between Ⅳ grade and Ⅴ grade(P>0.05), and there were statistically significant differences in ADC values between the other levels of groups(P<0.05). There was no statistically significant difference in eADC value between groups except for Ⅰ grade and Ⅱ grade(P>0.05), and there were statistically significant differences among other groups(P<0.05). Correlation analysis showed that ADC value was negatively correlated with Pfirrmann grading(r=-0.752, P<0.05), and eADC was positively correlated with Pfirrmann grading(r=0.736, P<0.05). Conclusion ADC and eADC values were highly correlated with Pfirrmann grade, and the changes of ADC and eADC values of lumbar intervertebral disc can accurately reflect the degree of lumbar disc degeneration.
