麻醉专业住院医师临床带教模式的思考与探讨 被引量:8

摘要 麻醉专业住院医师规范化培训(简称住培)是医学生毕业后进入工作岗位前再教育的关键阶段。经过此阶段的学习,大多数麻醉专业住院医师的基础理论知识、临床操作技能、科研和带教能力均有较大提升,从而为自己在以后能够独立从事临床麻醉工作奠定基础。但是,从一名刚毕业的医学生到一名合格麻醉住院医师的转变与临床的高质量带教是密不可分的。自住培制度实施以来,我们麻醉专业住院医师的培训质量逐年提高,但不可否认,我们的临床带教工作中仍然存在着诸多问题:(1)住培学员的麻醉基础理论和操作技能等诸方面的掌握程度不尽相同,且带教老师的临床带教能力也是参差不齐;(2)在本专业科室培训期间没有固定的临床带教老师,从而无法完成系统、规范的学习和培训;(3)在非本专业科室轮转期间缺乏管理,浮于表面,流于形式;(4)考核机制不明确,权责不明,没有奖罚落实到个人。因此,我们作为首批国家住培基地对多年来麻醉专业住院医师的临床带教进行了思考和总结,认为通过分层教学、一体化管理、多元化考核,可使麻醉专业住培医师形成严谨的工作态度、较强的工作能力、一定的临床带教和科研能力,从而提高麻醉住培质量。 The standardized training for anesthesia resident physician is a key stage for medical students to re-educate before entering the workplace after graduation.A qualified anesthesiologist should not only have comprehensive basic theoretical knowledge and skilled clinical operation techniques,but also have noble medical ethics,clinical teaching and scientific research ability.However,qualified anesthesiology resident training is inextricably linked to high quality clinical instruction.Since the implementation of the resident training system,the quality of anesthesia residency training increase year by year,but it is inevitable that the anesthesia residency training still exist many problems in clinical teaching work:(1)Anesthesia resident physician basic theory of anesthesia and operation skills is not the same,the teacher’s clinical teaching ability is uneven;(2)There is no fixed clinical teacher during the training period of the anesthesiology department,so it is impossible to complete the systematic and standardized learning and training;(3)Lack of management during the rotation of non-anesthesiology department;(4)The assessment mechanism is not clear,power and responsibility is unclear,no rewards and punishments are implemented to individuals.Therefore,we,as the first national standardized training base for resident physicians,thinking and summary the clinical teaching question of anesthesia,through hierarchical teaching,integrated management and diversified assessment,anesthesiological residents in standardized training have formed rigorous working attitude,strong working ability and certain clinical teaching and research ability,so as to improve the quality of anesthesia residency training.
