摘要
目的探讨卡前列素氨丁三醇注射液联合改良式多U型子宫压迫缝合术在高龄产妇产后出血中的应用效果。方法选取2016年9月至2019年10月本院收治的70例产后出血的高龄产妇作为研究对象,随机分为研究组和对照组,每组35例。对照组采用卡前列素氨丁三醇注射液联合常规B-Lynch缝合法止血,研究组采用卡前列素氨丁三醇注射液联合改良式多U型子宫压迫缝合术止血,比较两组围手术期指标(手术时间、术中出血量、术后24 h出血量、输注血浆量、住院时间)、应激反应指标[去甲肾上腺素(NE)、丙二醛(MDA)、内皮素-1(ET-1)]、激素指标[雌二醇(E_(2))和卵泡刺激素(FSH)]水平及并发症(月经异常、子宫切除、发热、继发性贫血)发生情况。结果研究组手术时间、住院时间均短于对照组,术中出血量、术后24 h出血量、输注血浆量均少于对照组(P<0.05)。治疗后,两组NE、MDA、ET-1水平均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组E_(2)和FSH水平比较差异无统计学意义。研究组并发症发生率为22.86%,低于对照组的45.71%,差异有统计学意义(P<0.05)。结论卡前列素氨丁三醇注射液联合改良式多U型子宫压迫缝合术对高龄产妇产后出血的止血效果显著,且能明显降低术后并发症发生率。
Objective To investigate the application effect of carboprost tromethamine injection combined with modified multi-U-shaped uterine compression suture in postpartum hemorrhage of elderly puerperae.Methods 70 elderly puerperae with postpartum hemorrhage admitted to our hos-pital from September 2016 to October 2019 were selected as the research subjects,and they were randomly divided into the study group and the con-trol group,with 35 cases in each group.The control group was treated with carboprost tromethamine injection combined with conventional B-Lynch suture for hemostasis,while the study group was treated with carboprost tromethamine injection combined with modified multi-U shaped uterine compression for hemostasis.The perioperative indexes(operation time,intraoperative blood loss,postoperative 24 h blood loss,plasma infusion,and hospitalization time),stress response indexes(norepinephrine[NE],malondialdehyde[MDA],and endothelin-1[ET-1]),hormone indexes(estradiol[E_(2)],and follicle stimulating hormone[FSH]),and complications(menstrual abnormalities,hysterectomy,fever,and secondary anemia)were com-pared between the two groups.Results The operation time and hospitalization time of the study group were shorter than those of the control group,and the intraoperative blood loss,postoperative 24 h blood loss and plasma transfusion were less than those of the control group(P<0.05).After treatment,the levels of NE,MDA and ET-1 in the two groups were lower than those before treatment,and the study group was lower than the control group(P<0.05).There was no significant difference in E_(2) and FSH levels between the two groups after treatmen.The complication rate in the study group was 22.86%,which was lower than 45.71%in the control group,and the difference was statistically significant(P<0.05).Conclusion The hemostatic effect of carboprost tromethamine injection combined with modified multi-U uterine compression suture on postpartum hemorrhage in el-derly parturients is remarkable and can significantly reduce the incid
