摘要
目的探讨正常胎儿脑干常用磁共振成像(MRI)生物学指标与孕周的相关性,建立MRI评估正常胎儿脑干发育的量化指标。方法收集2016年1月-2021年9月兰州大学第二医院产前胎儿脑MRI资料153例进行回顾性分析。根据T2加权像正中矢状位测量胎儿脑干各部分高度、前后径及截面积,分析与孕周的相关性。结果Pearson相关分析表明,胎儿脑干各部分高度、前后径及截面积与孕周均呈正相关关系,其中截面积的相关性最强,以桥脑最显著(r=0.963,P<0.05)。孕中晚期,桥脑的各项生物学指标增长速度最快、增长范围最大。小于孕28周,桥脑高度增长速度最快且大于前后径;大于孕28周,桥脑前后径增长速度最快且大于高度。结论胎儿脑干各部分高度、前后径及截面积与孕周明显呈正相关关系,经T2加权像正中矢状位测量脑干常用生物学指标可作为评估胎儿脑干生长发育的方法。
Objective To discuss the correlation between commonly magnetic resonance imaging(MRI)biological indexes of normal fetal brain stem and gestational age,to establish quantitative indexes for MRI evaluation of normal fetal brain stem development.Methods 153 cases of prenatal fetal brain MRI were collected and analyzed retrospectively from January 2016 to September 2021.The indicators according to the median sagittal position of T2-weighted image included the height,anteroposterior diameter and cross-sectional area of the midbrain,pons,medulla oblongata,as well as the anteroposterior diameter of tegmentum.The correlation analysis was performed to assess relationship between gestational age and all measurements.Results Pearson correlation analysis showed that the height,anteroposterior diameter and cross-sectional area of fetal brainstem components were positively correlated with the gestational age.The correlation between pons cross-sectional area and gestational age was the highest(r=0.963,P<0.05).In the middle and late pregnancy,the growth rate of pons was the fastest and the growth range was the largest.In gestation of less than 28weeks,the growth rate of pons height was the fastest and greater than the anterior posterior diameter;in gestation of more than 28 weeks,the anterior posterior diameter of pons increased the fastest and greater than the height.Conclusion The height,anteroposterior diameter and cross-sectional area of fetal brain stem are positively correlated with the gestational age.The common biological indexes of brain stem measured by T2weighted image in midsagittal position can be used as a method to evaluate the growth and development of fetal brain stem.
