摘要
目的:对儿童青少年身体活动与近视发病、进展的关系进行系统评价和Meta分析,为我国儿童青少年近视的防控和干预提供理论参考依据。方法:在WOS核心合集、PubMed、EBSCOhost、ScienceDirect、Cochrane Library、中国知网等数据库中,检索截至2019年8月25日的关于儿童青少年身体活动与近视关系的中英文文献,由2名研究者独立对纳入文献进行筛选、数据提取及质量评价,应用Stata 12.0软件进行Meta分析。结果:共30项研究纳入Meta分析,其中正视儿童青少年身体活动时长与近视发病关系的研究24项,近视儿童青少年身体活动时长与近视进展关系的研究6项。研究结果显示,正视儿童青少年身体活动时长与近视发病呈负相关关系(OR=0.90;95%CI:0.86,0.94;P=0.000;I2=85.1%),近视儿童青少年身体活动时长与近视进展无相关关系(OR=0.99;95%CI:0.93,1.05;P=0.000;I2=81.5%)。结论:正视儿童青少年身体活动时长与近视发病呈负相关关系,近视儿童青少年身体活动时长与近视进展无相关关系,但后续还需要更多高质量研究,尤其是干预研究来验证结果的可靠性。
Objective: Systematic review and Meta-analysis were used to do the comprehensive assessment to determine the association between physical activity(PA) and incidence and progression of myopia in children and adolescents, which provides evidence for effective preventions and interventions of myopia in Chinese children and adolescents. Methods: Both Chinese and English literature on the association between PA and myopia in children and adolescents as of August 25, 2019 were retrieved in the databases of WOS, PubMed, EBSCOhost, ScienceDirect, Cochrane Library, and CNKI. Two reviewers independently examined the articles, and performed data extraction and quality assessment. Then Meta-analysis was performed using Stata12.0 software. Results: A total of 30 studies were included in Meta-analysis, among which 24studies explored the association between the duration of PA and the incidence of myopia in children and adolescents with emmetropia, and the other 6 studies examined the association between the duration of PA and the progression of myopia in children and adolescents with myopia. The results showed that the more time spent on PA will lead to a lower incidence of myopia in children and adolescents with emmetropia(OR=0.90;95% CI: 0.86, 0.94;P=0.000;I2=85.1%), but PA will not lead to a slower progression of myopia in children and adolescents with myopia(OR=0.99;95% CI: 0.93, 1.05;P=0.000;I2=81.5%). Conclusions: The more time spent on PA will lead to a lower incidence of myopia in children and adolescents with emmetropia,but PA will not lead to a slower progression of myopia in children and adolescents with myopia.Due to the limitations of the type and quantity of included studies, more high-quality studies are needed to verify the conclusions.
