VAP危险因子前馈控制对老年重症肺炎机械通气效果及炎症反应的影响研究 被引量:5

Study of the Effect of Feedforward Control of VAP Risk Factors on Mechanical Ventilation and Inflammatory Response in Elderly Patients with Severe Pneumonia

摘要 目的:研究呼吸机相关性肺炎(VAP)危险因子前馈控制在老年重症肺炎患者机械通气护理中的应用效果。方法:将80例行机械通气治疗的老年重症肺炎患者随机分为对照组和观察组各40例,对照组采用常规护理,观察组采用常规护理+VAP危险因子前馈控制干预,对比两组患者VAP的发生率,并检测机械通气指标(PaO_(2)、PCO_(2)、OI)、血清炎症反应指标(PCT、CRP、IL-6)和呼吸功能指标(气道阻力、气道峰压、呼吸做功)。结果:观察组患者VAP发生率为5.00%,低于对照组的17.50%(P<0.05)。护理后,观察组患者PaO_(2)、OI均高于对照组(P<0.05),PCO_(2)及血清PCT、CRP、IL-6均低于对照组(P<0.05),观察组患者气道阻力、气道峰压、呼吸做功均低于对照组(P<0.05)。结论:老年重症肺炎患者行机械通气治疗中通过VAP危险因子前馈控制干预,能有效降低VAP发生率,提高机械通气效果和炎症反应的控制效果,改善患者呼吸功能。 Objective:To study the application effect of feedforward control of ventilator-associated pneumonia(VAP)risk factors in the mechanical ventilation nursing of elderly patients with severe pneumonia.Methods:80 elderly patients with severe pneumonia treated with mechanical ventilation were randomly divided into control group(n=40)and observation group(n=40),and the two groups were given routine nursing and routine nursing+VAP risk factor feedforward control intervention,respectively.The incidence of VAP in the two groups was compared,and mechanical ventilation indicators(PaO_(2),PCO_(2),OI),serum inflammatory response indicators(PCT,CRP,IL-6)and respiratory function indicators(airway resistance,airway peak pressure,respiratory work)were detected.Results:The incidence of VAP in observation group was 5%,lower than 17.5%in control group(P<0.05).After nursing,PaO_(2)and OI in observation group were higher than those in control group(P<0.05),PCO_(2),serum PCT,CRP and IL-6 were lower than those in control group(P<0.05),airway resistance,airway peak pressure and respiratory work in observation group were lower than those in control group(P<0.05).Conclusion:Feedforward control intervention of VAP risk factors in elderly patients with severe pneumonia undergoing mechanical ventilation can effectively reduce the incidence of VAP,improve the control effect of mechanical ventilation and inflammatory response,and improve the respiratory function of patients.
