摘要
目的 研究长期暴露洗涤剂引发接触性皮炎人群较正常人群的皮肤差异以及使用护手霜干预前后的改变,为长期暴露洗涤剂引发的皮肤伤害预防及治疗提供依据。方法 于2019年12月采用现场招募志愿者的方法,共招募64名18—28岁成年人,对其手部虎口处的水分含量及经皮水分流失(transepidermal water loss,TEWL)等生理指标进行测定。采用独立样本t检验判断符合接触性皮炎标准人群与正常人群指标的差异。随后对15名接触性皮炎患者进行护手霜干预,使用重复测量方差分析判断干预一周和两周后生理指标的变化情况。结果 干预前长期暴露组水分含量(11.88±5.80 au)较健康对照组(20.45±6.97 au)低8.57 au(t=6.110,P<0.001);TEWL值[24.97±14.73 g/(m^(2)·h)]较健康对照组[(13.19±6.86 g/(m^(2)·h)]高11.78 g/(m^(2)·h)(t=-4.244,P<0.001)。干预一周后,长期暴露人群水分含量(13.04±4.63 au)低于健康人群7.41 au (t=6.727,P<0.001),TEWL值[26.66±12.79 g/(m^(2)·h)]高于健康人群13.47 g/(m^(2)·h)(t=-5.534,P<0.001);干预两周后长期暴露人群水分含量(18.49±6.38 au)与健康人群无统计学差异(t=1.438,P=0.173),TEWL值[23.24±10.67 g/(m^(2)·h)]高于健康人群10.05 g/(m^(2)·h)(t=-4.864,P<0.001);此外,观察到长期暴露人群水分含量随着干预时间的延长,含量逐渐升高[干预一周后水分含量升高1.16 au,干预两周后升高6.61 au (F=13.921,P<0.001)]。结论 长期暴露洗涤剂导致接触性皮炎人群较健康人群手部屏障功能受到伤害。工作间歇使用护手霜可有效增加手部水分含量,略微降低经皮水分流失。建议长期暴露洗涤剂人群坚持使用护手霜以修复受损的皮肤屏障。
Objective To study development of contact dermatitis from chronic exposure to detergent among people with different skin conditions, to determine changes from interventions with hand cream, and to provide prevention and treatment protocols. Methods A total of 64 adults aged 18-28 years were recruited in December 2019 using an on-site volunteer recruitment method to measure physiological indicators of moisture content and transepidermal water loss(TEWL) in the tiger area of their hands. An independent sample t-test was used to determine differences in indicators between those who met the criteria for contact dermatitis and those who were normal. Hand cream intervention was then administered to 15patients with contact dermatitis, and changes in physiological indicators after one and two weeks of intervention were determined using repeated measures ANOVA. Results Moisture content was 8.57 au lower(t=6.110, P<0.001) in the longterm pre-intervention exposure group(11.88±5.80 au) compared to the healthy control group(20.45±6.97 au);TEWL values were 11.78 g/(m^(2)· h) [24.97±14.73 g/(m^(2)· h)] higher compared to the healthy control group [13.19±6.86 g/(m^(2)· h)](t=-4.244,P<0.001). One week after the intervention, the moisture content of the chronically exposed population(13.04±4.63 au) was lower than that of the healthy population by 7.41 au(t=6.727, P<0.001) and the TEWL value [26.66±12.79 g/(m^(2)· h)] was higher than that of the healthy population by 13.47 g/(m^(2)· h)(t=-5.534, P<0.001). Two weeks after the intervention, the water content of exposed population(18.49 ± 6.38 au) was not statistically different from that of the healthy population(t=1.438,P=0.173), and the TEWL value [(23.24 ± 10.67 g/(m^(2)· h)] was higher than that of the healthy population 10.05 g/(m^(2)· h)(t=-4.864, P<0.001). Furthermore, it was observed that the water content of the exposed population gradually increased with the increase of the intervention time(t=-4.864, P<0.001).
