系统性红斑狼疮患儿外周血p38MAPK、NF-κB表达水平的变化及临床意义

摘要 目的研究系统性红斑狼疮(SLE)患儿外周血p38MAPK、NF-κB表达水平的变化及临床意义。方法选择2018年6月至2020年12月期间成都市妇女儿童中心医院收治的94例SLE患儿并根据SLEDAI评分分别为SLE稳定组(n=52)、SLE活动组(n=42),另取同期体检的60例健康儿童作为对照组。检测外周血p38MAPK、NF-κB的表达水平、血沉(ESR)水平以及血清C反应蛋白(CRP)、补体3(C3)、补体4(C4)、白介素(IL)-1β、IL-6、Bcl-2相关x蛋白(Bax)、含半胱氨酸的天冬氨酸蛋白水解酶-3(Caspase-3)水平。结果SLE活动组外周血p38MAPK、NF-κB的表达水平高于SLE稳定组及对照组,差异有统计学意义(P<0.05);外周血p38MAPK、NF-κB的表达水平对SLE病情活动具有诊断价值,截断值分别为1.115、0.955;SLE活动组中有面部红斑、合并关节炎、合并LN患者的外周血p38MAPK、NF-κB表达水平均高于无面部红斑、无关节炎、无LN患者,差异有统计学意义(P<0.05);SLE活动组中外周血p38MAPK表达≥1.115、NF-κB表达≥0.955患者的CRP、ESR、IL-1β、IL-6、Bax、Caspase-3水平均高于外周血p38MAPK表达<1.115、NF-κB表达<0.955患者,C3、C4水平均低于外周血p38MAPK表达<1.115、NF-κB表达<0.955患者,差异有统计学意义(P<0.05)。结论SLE患儿外周血p38MAPK、NF-κB表达水平增加与病情活动、组织损伤加重有关。并激活炎症反应及细胞凋亡可能是与之相关的分子机制。 Objective To investigate the changes of p38MAPK and NF-κB expression levels in peripheral blood of children with systemic lupus erythematosus(SLE)and its clinical significance.Methods Total of 94 children with SLE admitted to Chengdu Women and Children’s Central Hospital from June 2018 to December 2020 were selected and divided into the SLE stable group(n=52)and the SLE active group(n=42)according to the SLEDAI score,and 60 healthy children who underwent physical examination during the same period were selected as the control group.The expression levels of p38MAPK and NF-κB in peripheral blood,erythrocyte septate(ESR),and serum C-reactive protein(CRP),complement 3(C3),complement 4(C4),interleukin(IL)-1β,IL-6,Bcl-2 associated X protein(Bax)and cysteine-containing aspartate protease-3(Caspase-3)were detected.Results The expression levels of p38MAPK and NF-κB in peripheral blood of the SLE active group were higher than those of the SLE stable group and the control group(P<0.05).The expression levels of p38MAPK and NF-κB in peripheral blood had diagnostic value for the disease activity of SLE,and the cut-off values were 1.115 and 0.955 respectively.The expression levels of p38MAPK and NF-κB in peripheral blood of patients with erythema facialis,arthritis and LN in the SLE active group were higher than those of patients without erythema facialis,arthritis and LN(P<0.05).The levels of CRP,ESR,IL-1β,IL-6,Bax and Caspase-3 in patients with p38MAPK expression≥1.115 and NF-κB expression≥0.955 in peripheral blood in SLE active group were higher than those in patients with p38MAPK expression<1.115 and NF-κB expression<0.955,and the levels of C3 and C4 were lower than those in patients with p38MAPK<1.115expression and NF-κB<0.955 expression(P<0.05).Conclusion The increased expression levels of p38MAPK and NF-κB in peripheral blood of children with SLE are related to disease activity and tissue injury.Activation of inflammatory response and cell apoptosis may be the related molecular mechanisms.
