摘要
目的系统评价活血化瘀类方药联合羟苯磺酸钙治疗糖尿病视网膜病变的有效性和安全性。方法计算机检索中国知网数据库(CNKI)、万方数据库(WF)、维普中文科技数据库(VIP)、PubMed、Embase、Cochrane Library等数据库中关于活血化瘀类方药联合羟苯磺酸钙治疗糖尿病视网膜病变的随机对照试验。检索时间从从建库到2020年2月,对纳入文献进行质量评价,并采用RevMan 5.3进行Meta分析。结果共检索出504篇文章,最终纳入17篇,总样本量为1,379例,全部为中文文献。Meta分析结果显示,活血化瘀类方药联合羟苯磺酸钙的临床疗效优于单用羟苯磺酸钙[RR=1.34,95%CI(1.23,1.45),P=0.000],在视力[MD=0.10,95%CI(0.08,0.12),P=0.000]、全血高切粘度[MD=-0.52,95%CI(-0.69,-0.36),P=0.000]、全血低切粘度[MD=-0.72,95%CI(-0.97,-0.47),P=0.000]、甘油三酯[MD=-0.15,95%CI(-0.24,-0.05),P=0.003]、胆固醇[MD=-0.22,95%CI(-0.31,-0.12),P=0.000]、血管内皮生长因子[MD=-20.88,95%CI(-35.41,-6.34),P=0.005]、有效降低不良反应发生率[MD=0.22,95%CI(0.05,0.91),P=0.040],与对照组比较,均有统计学意义。结论活血化瘀类方药联合羟苯磺酸钙治疗糖尿病视网膜病变的临床疗效更有优势,显著提高视力,改善血液流变学指标,一定程度上降低血脂、血管内皮生长因子的水平,且安全性相对高。
OBJECTIVE To systematically evaluate the efficacy and safety of prescriptions for promoting blood circulation and removing blood stasis combined with calcium dobesilate in the treatment of diabetic retinopathy.METHODS Randomized,controlled trials were retrieved from databases including Chinese National Knowledge Base(CNKI),Wanfang Database(WANFANG DATA),VIP Chinese Technology Database(VIP),PubMed,Embase,Cochrane Library and other databases by the computer on the application of prescriptions for promoting blood circulation and removing blood stasis combined with calcium dobesilate in the treatment of diabetic retinopathy.The retrieval time was from the establishment of the database to February 2020,the quality of the included literature was evaluated,and RevMan 5.3 was used for metaanalysis.RESULTS A total of 504 articles were retrieved,and 17 articles were finally included.The total sample size was 1,379 cases,all of which were Chinese literature.The results of meta-analysis showed that the clinical efficacy of prescriptions for promoting blood circulation and removing blood stasis combined with calcium dobesilate was better than that of calcium dobesilate alone[RR=1.34,95%CI(1.23,1.45),P=0.000],with improved visual acuity[MD=0.10,95%CI(0.08,0.12),P=0.000],reduced whole blood high shear viscosity[MD=-0.52,95%CI(-0.69,-0.36),P=0.000],reduced whole blood low shear viscosity[MD=-0.72,95%CI(-0.97,-0.47),P=0.000],lower level of triglyceride[MD=-0.15,95%CI(-0.24,-0.05),P=0.003],lower level of cholesterol[MD=-0.22,95%CI(-0.31,-0.12),P=0.000],lower level of vascular endothelial growth factor[MD=-20.88,95%CI(-35.41,-6.34),P=0.005].All the results were statistically significant,and the incidence of adverse reactions was significantly reduced[MD=0.22,95%CI(0.05,0.91),P=0.040].CONCLUSIONS Meta-analysis shows that the clinical efficacy of prescriptions for promoting blood circulation and removing blood stasis combined with calcium dobesilate in the treatment of diabetic retinopathy had more advantages,with significantly impro
