摘要
目的探求单相抑郁症患者中脑中缝核经颅超声(TCS)回声与帕罗西汀疗效的相关性。方法选择2015年3月至2017年1月金华市中心医院诊治的单相抑郁症患者52例,行中脑中缝核TCS检查,根据回声强度进行半定量分级,回声消失为1级,回声断续或中断为2级,回声连续为3级,其中回声1级、2级为低回声。均采用帕罗西汀治疗,分别于治疗前及治疗6周后行汉密尔顿抑郁量表17项(HAMD-17)评分,计算治疗前后的HAMD-17差值,分析中脑中缝核回声强度与HAMD-17差值的相关性。治疗6周后行临床总体印象改善量表(CGI-I)评分,分析中脑中缝核回声强度与CGI-I评分的相关性。结果中脑中缝核回声强度与HAMD-17差值及CGI-I评分均呈负相关(r=-0.563、-0.409,均P<0.05)。结论帕罗西汀对中脑中缝核低回声的单相抑郁症患者有较好的疗效。
Objective To explore the application of transcranial sonography(TCS)echo of the midbrain raphe nuclei in evaluating the efficacy of paroxetine for treatment of patients with unipolar depression.Methods Fifty-two patients with unipolar depression admitted in Jinhua Central Hospital from March 2015 to January 2017 were enrolled.All patients underwent TCS examination of the midbrain raphe nucleus,the echo intensity was semi-quantitative graded as:grade 1(the echo disappeared),grade 2(the echo intermittent or interrupted)and grade 3(the echo continuous),and grade 1 and grade 2 were hypoechoic.All patients were treated with paroxetine.The Hamilton Depression Rating Scale 17(HAMD-17)scores were evaluated before and 6 weeks after treatment,the difference was calculated.The correlation between the echo intensity of the raphe nuclei in the midbrain and the difference of HAMD-17 was analyzed.After 6 weeks of treatment,the clinical global impression improvement scale(CGI-I)was assessed,and the correlation between the echo intensity of the midbrain raphe nucleus and the CGI-I score was analyzed.Results The echo intensity of the raphe nuclei was negatively correlated with HAMD-17 and CGI-I scores(r=-0.563 and-0.409,both P<0.05).Conclusion Paroxetine is better effective in the treatment of unipolar depression with hypoechoic mesencephalic raphe nuclei.
