血清MIP-1α、MCP-1水平与小儿细菌性脑膜炎病情严重程度及预后的相关性分析 被引量:4

Analysis of the correlation between serum MIP-1αand MCP-1 levels and the severity of and prognosis for bacterial meningitis in children

摘要 目的探究血清巨噬细胞炎性蛋白-1α(MIP-1α)和单核细胞趋化蛋白-1(MCP-1)与小儿细菌性脑膜炎(BM)病情严重程度的相关性,并分析二者与BM患儿预后的关系方法选取本院收治的BM患儿68例,分为轻度组27例(无昏迷和惊厥)和重度组41例(有意识障碍、昏迷、惊厥)。另选取39例疑似BM,但脑脊液检查无异常的非感染性疾病患儿作为对照组。ELISA法检测受试患儿的血清MIP-1α、MCP-1水平,采用Pearson法分析血清MIP-1α与MCP-1的相关性,ROC曲线分析血清MIP-1α、MCP-1对BM患儿严重程度的预测价值,并分析血清MIP-1α、MCP-1与BM患儿不良预后的关系。结果轻度组、重度组BM患儿血清MIP-1α、MCP-1水平均高于对照组(均P<0.05),重度组血清MIP-1α、MCP-1水平高于轻度组(均P<0.05)。BM患儿血清MIP-1α水平与MCP-1呈正相关(r=0.312,P<0.05)。血清MIP-1α、MCP-1及二者联合预测BM严重程度的曲线下面积分别为0.834、0.837和0.921,敏感度分别为75.6%、82.9%和87.8%,特异度分别为81.5%、77.8%和88.9%。预后不良患儿血清MIP-1α、MCP-1水平均高于预后良好患儿(均P<0.05),且MIP-1α高表达组不良预后发生率高于MIP-1α低表达组(P<0.05),MCP-1高表达组不良预后发生率高于MCP-1低表达组(P<0.05)。结论血清MIP-1α、MCP-1水平随着BM患儿疾病严重程度加剧而增加,且血清MIP-1α、MCP-1高表达的BM患儿预后不良发生率较高。 Objective To explore the correlation between serum macrophage inflammatory protein-1α(MIP-1α),monocyte chemoattractant protein-1(MCP-1)and the severity of bacterial meningitis(BM)in children,and to analyze the relationship between the levels of the two and the prognosis of children with BM.Methods Subjects were a total of 68 children with BM seen at this Hospital,and the children were divided into 2 groups,those with mild BM(27 patients with no coma or convulsions)and those with severe BM(41 patients with altered consciousness,coma,and convulsions).Thirty-nine children suspected of having BM but who did not have an infectious disease(results of a cerebrospinal fluid examination were normal)served as the control group.The levels of serum MIP-1αand MCP-1 in the studied children were determined using ELISA,the correlation between serum MIP-1αand MCP-1 was analyzed using Pearson’s method,the predictive value of serum MIP-1αand MCP-1 at predicting the severity of BM in children was analyzed using the ROC curve,and the relationship between serum MIP-1αand MCP-1 levels and a poor prognosis for children with BM was analyzed.Results The levels of serum MIP-1αand MCP-1 in children with mild or severe BM were higher than those in the control group(120.57±14.69 pg/ml and 146.37±21.48 pg/ml)(P<0.05 for all),and the levels of serum MIP-1α(452.66±59.31 pg/ml)and MCP-1(791.62±86.74 pg/ml)in children with severe BM were higher than those in children with mild BM(368.19±54.08 pg/ml,652.81±80.39 pg/ml)(P<0.05 for both).There was a correlation between serum MIP-1αand MCP-1 in children with BM(r=0.312,P=0.010).When used to predict the severity of BM,serum MIP-1αhad an area under the curve of 0.834,a sensitivity of 75.6%,and a specificity of 81.5%.Serum MCP-1 had an area under the curve of 0.837,a sensitivity of 82.9%,and a specificity of 77.8%.A combination of the two had an area under the curve of 0.921,a sensitivity of 87.8%,and a specificity of 88.9%.The levels of serum MIP-1α(500.78±51.36 pg/ml)and MCP-1(857.3
