摘要
目的分析2017—2018年上海市宝山区登革热本地感染病例流行病学特征,评价防控措施效果,为登革热有效防治提供经验依据。方法采用现场流行病学方法,对2017—2018年上海市登革热本地感染病例开展流行病学调查,采集病例血清标本进行病毒核酸和抗原检测;采用布雷图指数(BI)法和双层叠帐法评价防蚊灭蚊效果。于2019年采集疫点健康人群血清标本检测IgG抗体。结果2017年9月25日报告1例上海市登革热本地感染病例,2018年8月1日、8月7日、8月9日相继报告3例登革热本地感染病例,4例病例均为登革热热病毒Ⅰ型,男性3例、女性1例;年龄中位数44岁(35~65岁)。2017年病例发生在宝山区友谊街道。2018年3例病例发生在宝山区淞南镇,3例病例居住地相距在200~1000 m。对4例病例核心区开展病例主动搜索,共调查18250户居民、182家单位,累计52830人,搜索发现12例发热病人,均排除登革热。核心区、警戒区范围内采取杀灭成蚊、清除蚊媒孳生地,3周后BI均保持在控制线5以下;双层叠帐法白纹伊蚊叮咬指数均保持在控制线2只(/人·h)以下。健康人群180人份血清标本进行登革热IgG抗体检测,均为阴性。结论4例病例均为上海市登革热本地感染病例,采取综合防控措施后得到有效控制。
Objective The purpose of this study is to analyze the epidemiological characteristics of indigenous dengue cases in Baoshan District,Shanghai from 2017 to 2018,and to evaluate the effect of the prevention and control measures,and we provide experience basis for effective prevention and control of dengue fever.Methods Field epidemiological methods,epidemiological investigation was performed on the indigenous dengue cases in Shanghai from 2017 to 2018.Serum samples of the cases were collected for the detection of viral nucleic acids,antigen and antibody.The results of mosquito prevention and elimination were evaluated by the Breteau Index(BI)and the double mosquito net method.Serum samples of healthy people in the core area were collected for IgG antibody detection in 2019.Results On September 25th,2017,one case of indigenous dengue fever was reported in Shanghai.On August 1st,7th and 9th,2018,three cases of indigenous dengue fever were reported successively,and four cases were all dengue virus type I.There were 3 males and 1 female in 4 cases with median age of 44(range:35-65).In 2017,the case occurred in Youyi street of Baoshan District.In 2018,three cases occurred in Songnan Town,Baoshan District,and three cases lived between 200 m and 1000 m apart.Active case search was carried out in the core area of 4 cases.A total of 18250 residents and 182 units with a total of 52830 people were investigated,12 patients were found to have an elevated body temperature,but not dengue fever.After the elimination of adult mosquitoes and breeding sources in the core area and the warning area,the BI was decreased under 5,and the adult index of Aedes albopictus decreased under 2/h.And 0.00%(0/180)of the healthy people was positive dengue IgG.Conclusions All of the 4 cases were indigenous dengue fever in Shanghai,which were effectively controlled by comprehensive control measures.
