丙泊酚与右美托咪定分别联合舒芬太尼在重症机械通气患者程序化镇静中的临床效果 被引量:4

Clinical Effect of Propofol and Dexmedetomidine Combined with Sufentanil in Programmed Sedation of Patients with Severe Mechanical Ventilation

摘要 目的:探讨丙泊酚与右美托咪定分别联合舒芬太尼在重症机械通气患者程序化镇静中的应用效果。方法:选择2019年12月-2020年12月在佛山市高明区人民医院ICU科行机械通气的80例患者,按随机数字表法分为A组(40例)和B组(40例)。两组均行程序化镇静,A组给予丙泊酚联合舒芬太尼,B组给予右美托咪定联合舒芬太尼。比较两组治疗时间、氧化应激与炎症反应指标及不良反应发生情况。结果:B组机械通气时间、ICU停留时间均短于A组,用药后12、24、36 h时肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、β-内啡肽(β-EP)水平均低于A组,超氧歧化酶(SOD)均高于A组,差异有统计学意义(P<0.05);B组不良反应发生率低于A组,差异有统计学意义(P<0.05)。结论:在重症机械通气患者程序化镇静中使用右美托咪定联合舒芬太尼,可减轻氧化应激、炎症反应,减少不良反应发生,缩短治疗时间。 Objective:To investigate the effect of propofol and dexmedetomidine combined with sufentanil in programmed sedation of patients with severe mechanical ventilation.Method:A total of 80 patients receiving mechanical ventilation in ICU department of Foshan Gaoming District People’s Hospital from December 2019 to December 2020 were selected and divided into group A(40 cases)and group B(40 cases)according to random number table method.Both groups were treated with programmed sedation,with Propofol combined with Sufentanil in group A and Dexmedetomidine combined with Sufentanil in group B.The treatment duration,oxidative stress and inflammatory response indexes and the occurrence of adverse reactions were compared between the two groups.Result:The mechanical ventilation time and ICU stay time in group B were shorter than those in group A,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)andβ-endorphin(β-EP)were lower than those in group A at 12,24 and 36 h after medication,and superoxide dismutase(SOD)were higher than those in group A,the differences were statistically significant(P<0.05).The incidence of adverse reaction in group B was lower than that in group A,and the difference was statistically significant(P<0.05).Conclusion:Dexmedetomidine combined with Sufentanil can reduce oxidative stress and inflammatory reaction,reduce the occurrence of adverse reactions and shorten the treatment time in patients with severe mechanical ventilation during programmed sedation.
