摘要
目的分析宁夏县级公立中医院的运营效率及其影响因素,为改善县级公立中医院运营效率提供科学、合理的决策依据。方法以宁夏12家县级公立中医院为决策单位,采用DEA-BCC模型、Malmquist指数模型以及Tobit回归评价县级公立中医院2009—2020年的总体效率、效率的动态变化及探析效率的影响因素。结果各决策单元效率值普遍较低,有效年份较少;2009—2020年样本医院的全要素生产指数(TFP)为0.950,总体呈现下降趋势,其下降是技术效率下降与技术衰退共同作用的结果;回归结果表明县域内常住人口及财政补助收入对县级公立中医院效率具有显著的正向作用;资产负债率、住院病人次均医药费用与样本医院的效率显著负相关。结论政府应进一步加大对县级中医院的投入,在关注社会发展环境的基础上合理配置卫生资源,医院自身也要加强内部管理,逐步提升医疗服务质量,进而实现最优的投入产出以提升中医院的运营效率。
Objective To analyze the operation efficiency and influencing factors of Ningxia and provide scientific and reasonable decision-making basis for improving the operation efficiency of county public hospitals. Methods With 12 countylevel public hospitals of traditional Chinese medicine in Ningxia as decision-making units, DEA-BCC model, Malmquist index model and Tobit regression were used to evaluate the overall efficiency, dynamic changes of efficiency and analysis of the influencing factors of efficiency of 2009-2020. Results The efficiency value of each decision-making unit was generally low and less effective;the TFP was 0.950 in 2009-2020, the overall decline was the result of the combined effect of the decline in technical efficiency and technological decline;the regression results showed that the permanent population and financial subsidy income had significant positive effect on the efficiency of public hospitals;the debt-to-asset ratio, medical expenses per inpatient rate and the efficiency of the sample hospitals had a significant negative correlation. Conclusion The government should further increase the investment in the county-level hospitals of traditional Chinese medicine, reasonably allocate health resources on the basis of paying attention to the social development environment, the hospital itself should also strengthen the internal management, gradually improve the quality of medical services, and then realize the optimal input and output to improve the operation efficiency of traditional Chinese medicine hospitals.
