基于RANKL/RANK/OPG通路研究绝经后老年骨质疏松患者骨代谢与肠道菌群失调的关系 被引量:5

摘要 目的基于RANKL/RANK/OPG通路研究绝经后老年骨质疏松患者骨代谢与肠道菌群失调的关系。方法选取绝经后骨质疏松老年女性患者177例为研究对象,以双歧杆菌/肠杆菌比值将患者分为研究组和对照组,采用酶联免疫吸附法检测血清炎症因子白细胞介素6(interleukin-6,IL-6)、肿瘤坏死因子α(tumor necrosis factor-α,TNF-α)、转化生长因子β1(transforming growth factor-β1,TGF-β1)及骨代谢指标骨钙素(osteocalcin,OC)、骨特异性碱性磷酸酶(bone-specific alkaline phosphatase,BALP)、25-羟维生素D[25-hydroxyvitamin D,25(OH)D]、抗酒石酸盐酸性磷酸酶5b(tartrate resistant acid phosphatase-5b,TRACP-5b)水平,采用Western-blot法检测关节液中核因子κB受体活化因子配体(receptor activator of nuclear factor-κB ligand,RANKL)、核因子κB受体活化因子(receptor activator of nuclear factor-κB,RANK)、骨保护素(osteoprotegerin,OPG)蛋白相对表达量。结果研究组患者血清IL-6、TNF-α、TGF-β1、OC、BALP、TRACP-5b水平明显高于对照组,血清25(OH)D水平低于对照组(P<0.05)。研究组患者关节液中RANKL、RANK蛋白相对表达量高于对照组,OPG蛋白相对表达量低于对照组(P<0.05)。相关性分析结果显示,研究组双歧杆菌与血清25(OH)D水平呈正相关(r=0.332,P<0.05),与血清OC、BALP和TRACP-5b水平呈负相关(r=-0.318、-0.408、-0.371,P<0.05);肠杆菌与血清25(OH)D水平呈负相关(r=-0.378,P<0.05),与血清OC、BALP和TRACP-5b水平呈正相关(r=0.356、0.343、0.364,P<0.05)。结论肠道菌群失调能够通过RANKL/RANK/OPG通路影响骨代谢,导致绝经后老年女性骨代谢异常和炎症因子水平升高。 Objective To study the relationship between bone metabolism and intestinal flora imbalance in elderly patients with postmenopausal osteoporosis based on RANKL/RANK/OPG pathway.Methods A total of 177 elderly female patients with postmenopausal osteoporosis were selected as the research subjects.The patients were divided into a research group and a control group based on the ratio of bifidobacteria to enterobacteria.Enzyme-linked immunosorbent assay(ELISA)was used to detect the serum inflammatory factors interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),transforming growth factor-β1(TGF-β1)and bone metabolism indexes osteocalcin(OC),bone-specific alkaline phosphatase(BALP),25-hydroxyvitamin D[25(OH)D],and tartrate resistant acid phosphatase-5b(TRACP-5b)levels.Western blot method was used to detect relative expression of receptor activator of nuclear factor-κB ligand(RANKL),receptor activator of nuclear factor-κB(RANK),osteoprotegerin(OPG)protein in the synovial fluid of patients.Results Serum IL-6,TNF-α,TGF-β1,OC,BALP,and TRACP-5b levels were significantly higher in the research group than in the control group,while serum 25(OH)D levels were significantly lower than those in the control group(P<0.05).The relative expressions of RANKL and RANK proteins in the synovial fluid were higher in the research group than in the control group,and the relative expression of OPG protein was lower than that of the control group(P<0.05).Correlation analysis showed that there was a positive correlation between bifidobacteria and 25(OH)D levels(r=0.332,P<0.05),and negatively correlated with OC,BALP,and TRACP-5b levels(r=-0.318,-0.408,-0.371,P<0.05).Enterobacteria were negatively correlated with serum 25(OH)D levels(r=-0.378,P<0.05),and positively correlated with OC,BALP,and TRACP-5b levels(r=0.356,0.343,0.364,P<0.05).Conclusion Intestinal flora imbalance may affect bone metabolism through the RANKL/RANK/OPG pathway,leading to abnormal bone metabolism and increased levels of inflammatory factors in elderly postmenopausal wo
