摘要
目的:探讨全身成像三维量化(GI3DQ)法定量评估不同程度主动脉瓣反流(AR)的可行性及准确性。方法:纳入轻度以上AR患者119例,根据实时三维超声心动图(RT3DE)法测量的AR容积(ARVol)进行分级并分组:轻中度反流组(n=35),中重度反流组(n=41),重度反流组(n=43)。使用GI3DQ法测量AR最大反流束体积代表ARVol,以RT3DE法测量的ARVol为参照标准。结果:(1)轻中度反流组,GI3DQ法与RT3DE法测量的ARVol之间呈正相关(r=0.39,P=0.0223);一致性分析:GI3DQ法较RT3DE法测量的ARVol平均低估8.62 ml,差异有统计学意义(P<0.01);(2)中重度及重度反流组,GI3DQ法和RT3DE法测量的ARVol之间相关系数分别为0.79和0.87(P均<0.0001),一致性分析差异均无统计学意义(P均> 0.05);(3)在轻中度、中重度及重度反流组,GI3DQ法和RT3DE法分级的一致率分别为:60.0%、85.4%、95.3%,中重度及重度反流组显著优于轻中度反流组(P均<0.05),中重度和重度反流组之间差异无统计学意义(P>0.05)。结论:对于轻中度AR患者,GI3DQ法较RT3DE法明显低估ARVol;对于中重度及重度AR患者,GI3DQ法与RT3DE法测量的ARVol之间有良好的相关性和一致性。GI3DQ法可对初步诊断为轻中度以上AR的患者进行相对准确的定量评估和分级。
Objectives: To investigate the feasibility and accuracy of general imaging three-dimensional quantification(GI3DQ) in quantitative evaluation of aortic regurgitation(AR) with different degrees.Methods: 119 patients with more than a mild degree of AR were enrolled and divided into three groups: mild to moderate AR group(n=35), moderate to severe AR group(n=41) and severe AR group(n=43) according to the grading results based on the aortic regurgitant volume(ARVol) measured by real-time three-dimensional echocardiography(RT3DE). The maximum regurgitant jet volume was measured by GI3DQ as ARVol. The ARVol measured by RT3DE was used as the reference standard.Results:(1) In mild to moderate AR group, ARVol measured by GI3DQ had a weak correlation with ARVol measured by RT3DE(r=0.39, P=0.0223) and showed an obvious underestimation(mean difference=-8.62 ml, P<0.01).(2) ARVol measured by GI3DQ had good correlation with ARVol measured by RT3DE in both the moderate to severe AR and severe AR groups(r=0.79 and 0.87, respectively, both P<0.0001), and there was no significant difference between the ARVol measured by these two methods in moderate to severe AR and severe AR groups based on consistency analysis(both P>0.05).(3) The grading consistency between GI3DQ and RT3DE in these three subgroups were 60.0%, 85.4%, 95.3%, respectively. The grading consistency between GI3DQ and RT3DE in moderate to severe AR and severe AR groups were better than that in mild to moderate AR group(both P<0.05), while there was no significant difference between the grading consistency in moderate to severe AR and severe AR groups(P>0.05).Conclusions: For patients with mild to moderate AR, GI3DQ shows an obvious underestimation in the measurement of ARVol compared with RT3DE. For patients with moderate to severe AR or severe AR, ARVol measured by GI3DQ shows a good correlation and agreement with ARVol measured by RT3DE, GI3DQ can thus make relatively accurate quantification and grading of AR in patients with initial diagnosis of more than mild to m
