中-重度主动脉瓣反流患者的主动脉根部形态与冠状动脉血流速度的关系

摘要 目的:探讨中-重度主动脉瓣反流患者的冠状动脉血流速度变化,同时了解主动脉根部形态对冠状动脉血流速度的影响。方法:收集我院2018年8月至2019年5月期间中-重度主动脉瓣反流患者41例(反流组),术前行经食道三维超声心动图检查,通过半自动方法获得主动脉根部形态参数。同时选取20例主动脉瓣功能正常的院内就诊者作为对照组,两组均进行冠状动脉造影检查,利用校正TIMI血流计帧法观察冠状动脉血流速度。结果:与对照组比较,反流组校正左前降支、左回旋支及右冠状动脉的TIMI血流帧数增加,差异均有统计学意义(P均<0.05)。反流组的三支血管平均帧数与主动脉根部形态参数包括主动脉窦部、窦管交界的平均径(r=0.728、0.716)、面积(r=0.735、0.740)及周长(r=0.731、0.747)之间均存在较强的正相关(P均<0.001)。结论:对于中-重度主动脉瓣反流的患者,其冠状动脉血流速度慢于主动脉瓣正常人群,同时血流速度也随着主动脉根部形态的不断扩大而逐渐减慢。 Objectives:To observe the changes of coronary flow velocity in patients with moderate or severe aortic regurgitation,and explore the influence of aortic root anatomy on coronary flow velocity in these patients.Methods:Forty-one patients with moderate or severe aortic regurgitation were enrolled in this study.The threedimensional transesophageal echocardiography was performed in all patients and diameters of aortic root anatomy were analyzed by semi-automatic method.All patients completed coronary angiography and the coronary flow velocity was observed by corrected TIMI frame count.Twenty patients with normal echocardiography and coronary angiography results were selected as the control group.Results:Compared with the control group,the TIMI frame count of the left anterior descending branch,the left circumflex branch and the right coronary artery were significantly increased in aortic regurgitation group(all P<0.05).In the aortic regurgitation group,there was a positive correlation between the average frame count of the three vessels and diameters of aortic root anatomy,including the average diameter(r=0.728,0.716),area(r=0.735,0.740)and circumference(r=0.731,0.747)of the aortic sinus and the sinotubular junction(all P<0.001).Conclusions:The coronary flow velocity of patients with moderate or severe aortic regurgitation is slower than that of normal population and gradually slows down in proportion with the dilatation of aortic root.
