摘要
目的探讨自体血液回输在骨科手术患者中的应用效果。方法收集2019年1月至2020年12月于雅安市人民医院行骨科手术患者63例,年龄为33~66岁,其中男性34例,女性29例,所有患者术中均采用自体血回输,记录术中出血量、术中自体血回输量、术后1-7 d输血不良反应发生情况,比较术前及术后1d患者凝血常规、血常规指标、血钾、以及血乳酸水平等指标变化。结果术中平均出血量为(621.76±45.34)mL,平均输入自体血量为(441.37±44.58)m L。术后1-7 d均未出现明显输血相关不良反应。与术前相比,患者术后1 d血浆凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)有所上升,纤维蛋白原(fibrinogen,FIB)较术前有所下降,但差异无统计学意义(P>0.05);术后1 d红细胞压积(hematocrit,Hct)、血红蛋白(hemoglobin,Hb)较术前下降,差异有统计学意义(P<0.05);术后1 d血钾、血乳酸水平、血小板与术前相比无明显变化(P>0.05)。结论自体血回输对骨科手术患者血常规、凝血常规及血气、电解质影响较小,且无不良反应发生,具有临床应用价值。
Objective To explore the application of cell salvage in patients undergoing orthopedic surgeries.Methods Data were retrospectively collected from 63 patients(34 males and 29 females)aged 33 to 66 years who underwent an orthopedic surgery in Ya’an People’s Hospital from January 2019 to December 2020;all the patients had intraoperative cell salvage;intraoperative blood loss,the volume of intraoperative cell salvage,and the incidence of transfusion-associated adverse reactions 1 to 7 days after surgery were recorded;comparisons were made for changes in indices such as routine coagulation test results,routine blood test results,blood potassium,and blood lactic acid level 1 day after surgery.Results The mean intraoperative blood loss was 621.76±45.34 mL,and the mean volume of cell salvage was 441.37±44.58 mL.There were no obvious transfusion-associated adverse reactions 1 to 7 days after surgery.At 1 day after surgery,the plasma prothrombin time and activated partial thromboplastin time increased,while fibrinogen decreased,but the differences were not significant(P>0.05);hematocrit and hemoglobin decreased significantly(P<0.05);there were no significant changes in the levels of blood potassium,blood lactic acid,and platelet(P>0.05).Conclusion In patients undergoing orthopedic surgeries,cell salvage has a minor impact on routine blood test results,routine coagulation test results,blood gas,and electrolytes,with no adverse reactions observed.
