摘要
目的探讨大隐静脉高位结扎联合剥脱术(HLS)、超声引导射频消融术(RFA)和腔内激光闭合术(EVLA)对下肢静脉曲张(VVLE)患者预后及生活质量的改善程度。方法收集2018年3月至2021年3月在烟台毓璜顶医院行手术治疗的1590例VVLE患者的临床资料,按照手术方式不同将患者分为HLS组(n=282)、RFA组(n=813)和EVLA组(n=495)。比较3组患者术前至术后6个月的临床治疗效果、术后并发症发生情况、静脉临床严重程度评分(VCSS)及静脉功能不全的流行病学和经济研究-生活质量/症状(VEINES-QOL/Sym)评分。结果所有患者的手术成功率均为100%。术后6个月,3组患者的临床治疗总有效率比较,差异无统计学意义(P>0.05)。RFA组和EVLA组患者的并发症总发生率均明显低于HLS组患者,差异均有统计学意义(P<0.05)。术后6个月,RFA组和EVLA组患者的VCSS评分均低于HLS组患者,VEINES-QOL/Sym评分均高于HLS组患者,差异均有统计学意义(P<0.05)。结论HLS、RFA和EVLA均是VVLE的有效手术治疗方法,但与HLS相比,RFA和EVLA的手术创伤小,术后并发症少,术后恢复快,疗效显著。
Objective To investigate the improvement of prognosis and quality of life of patients with varicose veins of the lower extremity(VVLE)by high saphenous vein ligation and stripping(HLS),ultrasound-guided radiofrequency ablation(RFA),and endovenous laser ablation(EVLA).Method The clinical data of 1590 patients with VVLE who underwent surgical treatment in Yantai Yuhuangding Hospital from March 2018 to March 2021 were collected,and the patients were divided into the HLS group(n=282),the RFA group(n=813),and the EVLA group(n=495)according to different surgical methods.The clinical treatment effect,postoperative complications,venous clinical severity score(VCSS),and venous insufficiency epidemiological and economic study-quality of life/symptom(VEINES-QOL/Sym)scores were compared between the three groups before the surgery to 6 months after the surgery.Result The surgical success rate of all patients was 100%.Six months after operation,there was no significant difference in the total effective rate of the three groups(P>0.05).The total incidence of complications in RFA group and EVLA group was significantly lower than that in HLS group(P<0.05).Six months after operation,the VCSS of patients in RFA group and EVLA group were lower than those in HLS group,and the VEINES-QOL/Sym scores were higher than those in HLS group(P<0.05).Conclusion HLS,RFA and EVLA are all effective surgical interventions for VVLE.However,compared with HLS,RFA and EVLA have less surgical trauma,fewer postoperative complications,faster postoperative recovery,and better clinical effect.
