摘要
目的观察类风湿关节炎(rheumatoid arthritis,RA)患者血细胞衍生参数中性粒细胞/淋巴细胞比值(neutrophil-to-lymphocyte,NLR)、血小板/淋巴细胞比值(platelet-to-lymphocyte,PLR)、淋巴细胞/单核细胞比值(lymphocyteto-monocyte,LMR)、嗜中性白细胞与淋巴细胞比值(the derived neutrophil-to-lymphocyte,dNLR)、系统性免疫性炎症指数(systemic immune inflammation index,SII)、全身炎症反应指数(systemic inflammation response index,SIRI)、总炎症全身指数(aggregate inflammation systemic index,AISI)的变化及其与RA病情活动性的相关性。方法选取2019年9月—2020年9月本院就诊的RA患者110例作为RA组,另选取同期于本院体检的健康人群110名作为对照组。比较两组NLR、PLR、LMR、dNLR、SII、SIRI、AISI。根据病情严重程度将RA分为高、中、低度活动组,比较不同程度RA患者的NLR、PLR、LMR、dNLR、SII、SIRI、AISI。分析NLR、PLR、LMR、dNLR、SII、SIRI、AISI与28处关节疾病活动度评分(DAS28)的相关性。结果RA组的NLR、PLR、dNLR、SII、SIRI、AISI均高于对照组,而LMR低于对照组(P<0.001)。不同严重程度RA患者的NLR、PLR、LMR、dNLR、SII、SIRI、AISI比较,差异均有统计学意义(P<0.001)。NLR、PLR、dNLR、SII、SIRI、AISI与DAS28评分呈正相关(P<0.001),而LMR与DAS28评分呈负相关(P<0.001)。结论血细胞衍生参数对RA患者的诊断及严重程度判断具有重要意义,值得临床推广应用。
Objective To observe the changes of derived parameters of hemocytes in patients with rheumatoid arthritis(RA),including the changes of neutrophil-to-lymphocyteratio(NLR),platelet-to-lymphocyte ratio(PLR),lymphocyte-to-monocyte ratio(LMR),the derived neutrophil-to-lymphocyte ratio(dNLR),systemic immune inflammation index(SII),systemic inflammation response index(SIRI),aggregate inflammation systemic index(AISI)and their correlation with RA disease activity.Methods A total of 110 RA patients admitted to our hospital from September 2019 to September 2020 were selected as the RA group.Another 110 healthy people who underwent physical examination in our hospital were selected as the control group.NLR,PLR,LMR,dNLR,SII,SIRI and AISI were compared between two groups.The RA patients were divided into the high,medium and low activity groups according to the severity of the disease.NLR,PLR,LMR,dNLR,SII,SIRI and AISI were compared among patients with RA of different degrees of severity.This study also analyzed the correlation between NLR,PLR,LMR,dNLR,SII,SIRI and disease activity score 28(DAS28)was analyzed.Results RA groups had higher levels of NLR,PLR,dNLR,SII,SIRI,AISI and lower levels of LMR than the control group(P<0.001).There were statistically significant differences in NLR,PLR,LMR,dNLR,SII,SIRI and AISI among patients with RA of different severity(P<0.001).NLR,PLR,dNLR,SII,SIRI,AISI were positively correlated with the DAS28 score(P<0.001),while LMR was negatively correlated with the DAS28 score(P<0.001).Conclusion Derived parameters of hemocytes are of great significance for the diagnosis of RA and judgment of severity of RA in patients,thus worthy of clinical application.
