摘要
目的探讨肝癌患者入院时的中性粒细胞/淋巴细胞比值(NLR)、单核细胞/淋巴细胞比值(MLR)及红细胞分布宽度(RDW)在预测90 d死亡率上的价值。方法回顾性收集本院161例肝癌患者,其中90 d生存组119例,死亡组42例,分析NLR、MLR及RDW与肝功能、肿瘤标志物及巴塞罗那临床肝癌分期(BCLC)对90 d死亡率的预测价值。结果死亡组肝癌患者NLR、MLR及RDW较高(P<0.05)。NLR、MLR及RDW与肝功能、肿瘤标志物水平、BCLC分期呈正相关(P<0.05)。NLR、MLR及RDW单独预测肝癌患者入院90 d死亡率的曲线下面积(AUC)灵敏度及特异度分别为76.2%、78.2%;85.7%、58.8%;76.2%、77.3%,联合预测能提高灵敏度,但特异性降低。Kaplan-Meier生存曲线显示,NLR>5.479、MLR>0.380、RDW>15.08%的患者90 d的死亡率分别为55.2%、42.4%及54.2%。结论肝癌患者入院时的NLR、MLR及RDW水平与BCLC分期与90 d死亡率呈正相关。
Objective To explore the value of neutrophil/lymphocyte ratio(NLR),monocyte/lymphocyte ratio(MLR)and red blood cell distribution width(RDW)in predicting 90-day mortality in hepatocellular carcinoma(HCC)patients.Methods A total of 161 HCC patients in our hospital were retrospectively collected,including 119 patients in 90-day survival group and 42 patients in 90-day death group.The predictive value of NLR,MLR,RDW,liver function,tumor markers and Barcelona clinical liver cancer(BCLC)on 90-day mortality was analyzed.Results NLR,MLR and RDW were higher in the death group(P<0.05).NLR,MLR and RDW were positively correlated with liver function,tumor markers and BCLC stage(P<0.05).The area under the curve(AUC)showed the sensitivity and specificity of NLR,MLR and RDW were 76.2% and 78.2%,85.7% and 58.8%,76.2% and 77.3%,respectively.Combined prediction could improve the sensitivity but reduce the specificity;Kaplan-Meier survival curve showed that the 90-day mortality of patients with NLR>5.479,MLR>0.380 and RDW>15.08% were 55.2%,42.4% and 54.2%,respectively.Conclusion The levels of NLR,MLR and RDW on admission were positively correlated with BCLC stage and 90-day mortality.
