摘要
目的脑梗死卧床留置导尿管患者风险、护理需求,研究集束化护理干预对患者尿路感染、不良事件的影响。方法选择我院2019年9月至2020年5月收治的脑梗死卧床留置导尿管患者,总计90例。按照入院先后顺序分组,即常规护理的对照组(n=45)、集束化护理干预的观察组(n=45)。比较两组患者的生活质量评分、情绪评分等护理指标,以健康调查简表(SF-36)评估患者生活质量,以焦虑自评量表(SAS)、抑郁自评量表(SDS)评估患者的情绪状况。结果观察组、对照组脑梗死卧床留置导尿管患者尿路感染、漏尿、导尿管变形、引流不畅等不良事件发生情况经统计学计算,组间差异显著,P<0.05。观察组、对照组脑梗死卧床留置导尿管患者导管留置时间、SAS评分及SDS评分、满意度、生活质量评分经统计学计算,组间差异显著,P<0.05。结论集束化护理干预可以积极预防脑梗死卧床留置导尿管患者尿路感染、不良事件,提高患者满意度与生活质量、稳定患者情绪。
Objective To study the risk and nursing needs of patients with cerebral infarction and indwelling catheters,and to study the effect of clustered nursing intervention on patients with urinary tract infections and adverse events.Methods A total of 90 patients with cerebral infarction and indwelling catheters who were admitted to our hospital from September 2019 to May 2020 were selected.They were grouped according to the order of admission,namely the control group for routine care(n=45)and the observation group for clustered care intervention(n=45).The quality of life scores and emotional scores of the two groups of patients were compared.The MOS Item Short from Health Survey(SF-36)was used to assess the quality of life of the patients,and the Self-rating Anxiety Scale(SAS)and Slef-rating Depression Scale(SDS)assess the emotional state of patients.Results The incidence of adverse events such as urinary tract infection,urinary leakage,catheter deformation and poor drainage in patients with cerebral infarction in bed in the observation group and the control group was statistically calculated,and there were significant differences between the groups,P<0.05.The catheter indwelling time,SAS and SDS scores,satisfaction and quality of life scores of patients with cerebral infarction in the observation group and the control group were statistically calculated,and there were significant differences between the groups,P<0.05.Conclusion Clustered nursing intervention can actively prevent urinary tract infections and adverse events in patients with cerebral infarction and indwelling catheters,improve patient satisfaction and quality of life,and stabilize patients'emotions.
