社区骨密度筛查及和血糖、血脂及尿酸的相关性研究

摘要 目的 :了解虹口区北外滩社区老年人骨质疏松患病现状,并分析影响骨质疏松的相关因素,为社区早期发现及预防骨质疏松提供参考。方法 :选取1 477例参与上海北外滩社区体检的老年人进行骨密度测量,并分析年龄、性别、体质指数及血糖、血脂指标对骨密度的影响情况。结果 :共检出骨质疏松高危对象1 064例,检出率为72.04%,其中900例(60.93%)为骨量减少,164例(11.1%)为骨质疏松。女性高危人群和骨质疏松的检出率均高于男性(均P <0.05)。Spearman相关性分析显示高密度脂蛋白胆固醇、总胆固醇、尿酸水平与骨密度有相关性(r分别为-0.161,-0.077,0.185,均P <0.05),多因素Logistic回归分析显示年龄(OR=1.084,95%CI:1.055~1.114)、女性(OR=2.252,95%CI:1.48~3.425)是骨质疏松的危险因素,而尿酸(OR=0.996,95%CI:0.994~0.999)、体质指数(OR=0.866,95%CI:0.814~0.921)是维持骨量正常的有利因素。结论 :年龄、女性是老年人发生骨质疏松的危险因素,而尿酸,体质指数对老年人骨代谢发挥一定的保护作用。在开展社区居民骨质疏松筛查时应重点关注与以上因素相关的目标人群。 Objective: To investigate the prevalence of osteoporosis in the elderly in Beiwaitan Community of Hongkou District and analyze the related factors affecting osteoporosis to provide reference for early detection and prevention of osteoporosis in the community. Methods: One thousand four hundred and seventy-seven elderly people who participated in the community physical examination were selected for bone mineral density measurement in Beiwaitan Community of Shanghai, and the effects of age, sex, body mass index, blood glucose and blood lipid on bone mineral density were analyzed. Results: A total of 1 064 high-risk objects of osteoporosis were detected, and the detection rate was 72.04%, of which 900 cases(60.93%) were osteopenia and 164 cases(11.1%) were osteoporosis. The detection rates of high-risk groups and osteoporosis in women were higher than those in men(all P<0.05). Spearman correlation analysis showed that the levels of HDL cholesterol, total cholesterol and uric acid were correlated with bone mineral density(r were-0.161,-0.077 and 0.185, respectively, P<0.05), multivariate logistic regression analysis showed that age(OR=1.084, 95% CI: 1.055-1.114) and the female(OR=2.252, 95% CI: 1.48-3.425)were the risk factors of osteoporosis, Uric acid(OR=0.996, 95% CI: 0.994-0.999) and body mass index(OR=0.866, 95% CI:0.814-0.921) were favorable factors for maintaining normal bone mass. Conclusion: Age and female are the risk factors of osteoporosis in the elderly, and uric acid and body mass index play a certain protective role on bone metabolism in the elderly.When conducting osteoporosis screening in community residents, and the target population related to the above factors should be focused on.
