摘要
目的研究不同麻醉方式对剖宫产产妇母脐血中缺血修饰白蛋白(IMA)、白介素-2(IL-2)、白介素-6(IL-6)水平的影响。方法选取2019年4月至2021年4月于南阳市卧龙区妇幼保健院进行剖宫产手术的113例产妇,根据不同麻醉方式分为硬腰联合麻醉组(n=63,硬腰联合麻醉)和全身麻醉组(n=50,全身麻醉),比较两组麻醉效果、麻醉相关指标(麻醉起效时间、麻醉用药总剂量、清醒时间)、母脐血中IMA、IL-2、IL-6水平及不良反应。结果硬腰联合麻醉组优良率(93.65%)高于全身麻醉组(78.00%),麻醉起效时间、麻醉用药总剂量均少于全身麻醉组,差异有统计学意义(P<0.05)。两组清醒时间比较,差异无统计学意义(P>0.05)。两组母血IMA、IL-2、IL-6比较:术中30 min患者IMA、IL-6升高,且全身麻醉组高于硬腰联合麻醉组,IL-2下降,且全身麻醉组低于硬腰联合麻醉组,差异具有统计学意义(P<0.05);两组脐血IMA、IL-2、IL-6比较:全身麻醉组IMA、IL-6高于硬腰联合麻醉组,IL-2低于硬腰联合麻醉组,差异均有统计学意义(P<0.05)。硬腰联合麻醉组不良反应率(4.76%)低于全身麻醉组(16.00%),差异有统计学意义(P<0.05)。结论不同麻醉方式对剖宫产产妇母脐血中IMA、IL-2、IL-6的影响存在一定差异,相比全身麻醉,硬腰联合麻醉效果更为明显,对机体产生的应激反应更小,安全性更高。
Objective To study the effects of different anesthesia methods on ischemia modified albumin(IMA),interleukin-2(IL-2)and interleukin-6(IL-6)in maternal cord blood of cesarean section women.Methods A total of 113 puerperae who underwent cesarean section at the Maternal and Child Health Hospital of Wolong District,Nanyang City from April 2019 to April 2021 were selected.According to different anesthesia methods,they were divided into the study group(n=63,Combined spinal-epidural anesthesia)and the control group(n=50,general anesthesia),anesthesia effect,anesthesia-related indicators[including anesthesia onset time,total anesthetic dose,awake time],IMA,IL-2,IL-6 levels in maternal cord blood and adverse reactions of the two groups were compared.Results The excellent and good rate in the study group(93.65%)was significantly higher than that in the control group(78.00%),the onset time of anesthesia,and the total dose of anesthesia drugs were all lower than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the awake time between the two groups(P>0.05).Comparison of maternal blood IMA,IL-2,and IL-6 between the two groups:IMA and IL-6 increased in the 30 min intraoperative patients,and the general anesthesia group was higher than the epidural anesthesia group,IL-2 decreased,and the general anesthesia group was lower.In the epidural anesthesia group,the difference was statistically significant(P<0.05).The adverse reaction rate in the epidural anesthesia group(4.76%)was lower than that in the general anesthesia group(16.00%),and the difference was statistically significant(P<0.05).Conclusion There are certain differences in the effects of different anesthesia methods on IMA,IL-2,and IL-6 in maternal cord blood of cesarean section women,compared with general anesthesia,the combined spinal-epidural anesthesia is more obvious and the stress response to the body is less and higher security.
