摘要
目的探讨扩创植皮联合封闭负压引流技术(VSD)治疗大面积烧伤的效果。方法抽取2015年9月至2020年9月解放军联勤保障部队第九八九医院收治的62例大面积烧伤患者为研究对象,按照随机数字表法分为研究组与对照组,每组31例。对照组采用扩创植皮联合常规换药治疗,研究组采用扩创植皮联合VSD治疗。治疗1周后,比较两组的临床疗效;分析两组患者术后3 d时的植皮成活率及术后7 d时的创面愈合率;记录术后第1、3、7天两组患者创面细菌数量变化;比较两组术后1周的并发症发生情况。结果治疗1周后,研究组治疗总有效率(60.54%,20/31)高于对照组(38.71%,12/31),P<0.05。术后3 d,研究组植皮成活率(77.42%,24/31)高于对照组(51.61%,16/31),P<0.05;术后7 d,研究组创面愈合率(80.65%,25/31)高于对照组(54.84%,17/31),P<0.05。术后第1天,两组创面细菌数量比较差异未见统计学意义(P>0.05);术后第3、7天,研究组创面细菌数量均少于对照组(P均<0.05)。术后1周,研究组并发症总发生率(3.23%,1/31)低于对照组(19.35%,6/31),P<0.05。结论扩创植皮联合VSD治疗大面积烧伤,有助于提升患者的治疗效果、植皮成活率及创面愈合率,有效降低创面细菌数量及术后并发症发生率。
Objective To investigate the efficacy of debridement skin grafting combined with vacuum sealing drainage(VSD)in the treatment of large-area burns.Methods Sixty-two patients with large-area burns treated in NO.989 Hospital of the Joint Logistics Support Force of the People’s Liberation Army of China from September 2015 to September 2020 were selected as the research subjects,and they were divided into study group and control group according to the random number table method,with 31 cases in each group.Patients in control group were treated by debridement skin grafting combined with conventional dressing change,and patients in study group were treated by debridement skin grafting combined with VSD.After 1 week of treatment,the clinical efficacy of the two groups were compared.The 3-day postoperative survival rate of skin grafts and 7-day postoperative wound healing rate of the two groups were analyzed.The changes of wound bacteria in the two groups on the 1st,3rd and 7th day after operation were recorded.The occurrence of complications within 1 week after surgery of the two groups were compared.Results After 1 week of treatment,the total effective rate of study group(60.54%,20/31)was higher than that of control group(38.71%,12/31),P<0.05.Three days after operation,the survival rate of skin grafting in study group(77.42%,24/31)was higher than that in control group(51.61%,16/31),P<0.05.Seven days after surgery,the wound healing rate in study group(80.65%,25/31)was higher than that in control group(54.84%,17/31),P<0.05.On the first day after operation,there was no significant difference in the amount of wound bacteria between the two groups(P>0.05);on the 3rd and 7th day after operation,the amount of wound bacteria in study group was less than that in control group(all P<0.05).One week after surgery,the total incidence of complications in study group(3.23%,1/31)was lower than that in control group(19.35%,6/31),P<0.05.Conclusions Debridement skin grafting combined with VSD in the treatment of patients with large-area
