摘要
目的:观察益气养阴活血化瘀法治疗2型糖尿病合并甲状腺结节的临床疗效。方法:选取60例2型糖尿病合并甲状腺结节患者随机分为两组各30例,两组均予西医常规治疗,包括糖尿病健康宣教、低碘饮食、运动等,并予降糖药物治疗(二甲双胍除外),使血糖持续达标;治疗组另予益气养阴、活血化瘀中药治疗,两组疗程均为3个月,观察治疗后甲状腺结节最大径、促甲状腺激素(TSH)、血清游离三碘甲腺原氨酸(FT3)、血清游离甲状腺素(FT4)变化并评价疗效。结果:治疗组的甲状腺结节最大径治疗前后差异具有统计学意义(P﹤0.01),对照组治疗前后差异无统计学意义(P﹥0.05),两组TSH、FT3、FT4水平治疗前后差异均无统计学意义(P﹥0.05),治疗组临床总有效率63.3%高于对照组3.3%,差异有统计学意义(P﹤0.01)。结论:益气养阴、活血化瘀中药治疗2型糖尿病合并甲状腺结节临床疗效较好。
Objective:To observe the clinical effect of Chinese herbal medicine for tonifying qi,nourishing yin,activating blood circulation and removing blood stasis on type 2 diabetes mellites complicated with thyroid nodule.Methods:60 cases of selected patients with type 2 diabetes mellites complicated with thyroid nodule were randomly divided into two groups,30 cases in each group.Both groups were treated with conventional western medicine(including diabetes health education,low iodine diet,exercise,etc.and treated with hypoglycemic drugs except metformin),so as to keep blood sugar up to standard.The treatment group was treated plus with Chinese herbal medicine for tonifying qi,nourishing yin,activating blood circulation and removing blood stasis.The course of treatment was 3 months for both groups.After treatment,the changes of following indexes were observed such as maximum diameter of thyroid nodule,thyroid stimulating hormone(TSH),serum free triiodothyronine(FT3)and serum free thyroxine(FT4).And the curative effect was evaluated.Results:The difference of maximum diameter of thyroid nodule in treatment group before and after treatment was statistically significant(P ﹤ 0.01),while this difference in control group before and after treatment was not statistically significant(P ﹥ 0.05).The levels of TSH,FT3 and FT4 in the two groups before and after treatment were not statistically significant(P﹥0.05).The clinical total effective rate of treatment group was 63.3%,higher than 3.3% of control group,and the difference was statistically significant(P﹤0.01).Conclusion:Chinese herbal medicine for tonifying qi,nourishing yin,activating blood circulation and removing blood stasis has a better clinical effect on type 2 diabetes mellitus complicated with thyroid nodule.
