影响糖尿病患者自我管理效果的多因素分析及健康教育护理效果体会 被引量:3

摘要 目的:分析糖尿病患者自我管理效果的影响因素及健康教育的护理效果。方法:选取2018年1月—2020年4月间惠州市中大惠亚医院收治的240例糖尿病患者作为研究对象,通过双盲选法将其分为对照组和实验组,每组各120例,对照组进行常规护理,观察其护理后的自我管理效果,并且对自我管理效果的影响因素行单因素分析和多因素logistic回归分析,实验组在常规护理的基础上增加健康教育护理,比较两组患者护理后的自我管理行为评分以及血糖相关指标差异。结果:年龄以及性别和糖尿病患者的自我管理效果差异无统计学意义(χ^(2)=0.011、0.011,P>0.05)。文化水平、是否接受健康教育、支付方式、病程、有无住院史以及社会支持和糖尿病患者的自我管理效果,差异有统计学意义(χ^(2)=24.845、25.836、14.464、14.085、19.798、21.607,P<0.05)。多因素logistic回归分析结果显示,文化水平以及住院史是影响糖尿病患者自我管理效果的独立危险因素,而接受健康教育以及社会支持是影响糖尿病患者自我管理效果的保护因素,差异有统计学意义(P<0.05)。实验组患者的自我管理行为评分显著高于对照组,差异有统计学意义(χ^(2)=37.578,P<0.05)。实验组患者护理后的空腹血糖值以及糖化血红蛋白水平显著低于对照组,差异有统计学意义(χ^(2)=9.417、16.046,P<0.05)。结论:文化水平以及住院史是影响糖尿病患者自我管理效果的独立危险因素,而接受健康教育以及社会支持则为保护因素,对糖尿病患者开展健康教育护理可以进一步提高其自我管理行为,并且更有利于控制其血糖水平。 Objective:To analyze the factors affecting the effect of self-management of patients with diabetes and the nursing effect of health education.Methods:From January 2018 to April 2020,240 diabetic patients admitted to the hospital were selected as the research subjects.They were divided into control group(120 cases)and experimental group(120 cases)by doubleblind selection.The control group was given routine nursing care,and the self-management effect after nursing was observed,and single factor analysis and multi-factor logistic regression analysis were performed on the influencing factors of self-management effect.Health education nursing was added in the experimental group on the basis of routine nursing.The scores of selfmanagement behavior and blood glucose related indexes were compared between the two groups after nursing.Results:There was no statistically significant difference in self-management effect between age and gender and diabetes patients(χ^(2)=0.011,0.011,P>0.05).There were statistically significant differences in cultural level,whether to receive health education,payment methods,duration of illness,hospitalization history,social support and self-management effects of diabetic patients,the difference was statistically significant(χ^(2)=24.845,25.836,14.464,14.085,19.798,21.607,P<0.05).The results of multivariate logistic regression analysis showed that education level and hospitalization history were independent risk factors that affected the selfmanagement effect of diabetic patients,while health education and social support were protective factors that affected the selfmanagement effect of diabetic patients.The difference was statistically significant(χ^(2)=37.578,P<0.05).The self-management behavior score of the experimental group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).The fasting blood glucose and glycosylated hemoglobin levels of the experimental group after nursing were significantly lower than those of the control group,
