摘要
目的观察功能性鼻内镜手术(FESS)术后两性霉素B(AmB)鼻窦冲洗治疗真菌球型鼻窦炎(FBS)的临床疗效。方法选取2019年6月~2021年5月期间本院收治的50例行FESS的FBS患者,根据治疗方法不同,将患者分为治疗组和对照组每组25例,术后治疗组(n=25)予200μg/ml AmB行鼻窦内冲洗,对照组(n=25)予0.9%生理盐水冲洗,对患者术前、术后12周采用视觉模拟量表(VAS)、鼻腔鼻窦结局测试20(SNOT-20)、T&T嗅觉识别测试、鼻内镜下Lund-Kennedy评分、鼻窦CT Lund-Mackay评分法进行评分,并对患者术后12周的治疗效果进行分析,比较两组患者术后12周的临床疗效。结果两组术后12周的各项评分均低于术前,差异有统计学意义(P<0.05);术后12周治疗组的各项评分均优于对照组,差异有统计学意义(P<0.05)。结论FESS术后AmB鼻窦冲洗治疗FBS的临床疗效确切,值得临床推广应用。
Objective To investigate the clinical efficacy of sinus irrigation with amphotericin B(AmB)in the treatment of fungal ball sinusitis(FBS)after functional endoscopic sinus surgery(FESS).Methods Fifty FBS patients undergoing FESS in our hospital from June 2019 to May 2020 were selected.Patients were divided into treatment group and control group according to different treatment methods,with 25 patients in each group.After FESS,the treatment group(n=25)received sinus irrigation with 200μg/ml AmB,while the control group(n=25)was irrigated with 0.9%normal saline.The patients were scored using the visual analogue scale(VAS),sino-nasal outcome test-20(SNOT-20),T&T olfactory identification test,Lund-Kennedy endoscopic score and Lund-Mackay sinus CT score before and 12 weeks after surgery.In addition,the therapeutic effect of 12 weeks after surgery was analyzed,and the clinical efficacy of 12 weeks after surgery were compared between the two groups.Results The scores of the two groups at 12 weeks after surgery were all lower than those before surgery,and the scores of the treatment group were all superior to those of the control group at 12 weeks after surgery(P<0.05).Conclusion AmB sinus irrigation after FESS presents definite clinical efficacy in the treatment of FBS,so it is worthy of clinical promotion and application.
