基于mDIXON-Quant定量T_(2)^(*)值在腰椎间盘退行性变程度评估中的临床价值分析 被引量:3

摘要 目的探讨MRI mDIXON-Quant定量T_(2)^(*)值在腰椎间盘退行性变程度评估中的临床价值。方法在2019年3月~2019年11月招募年龄19~78岁,进行腰椎常规MRI及mDIXON-Quant检查,纳入标准且图像质量满意的共98例,平均年龄(42.2±17.1)岁。根据Pfirrmann标准将L_(1)/L_(2)~L_(5)/S_(1)椎间盘进行退行性变分级,并采用水-脂分离(mDIXON-Quant)技术定量测量相应椎间盘的T_(2)^(*)值。采用SPSS 19.0软件,运用一般线性方差t检验对不同检查者L_(1)~S_(1)椎间盘相应的T_(2)^(*)值与Pfirrmann分级进行比较,P<0.05为差异有统计学意义。用Pearson等级相关分析T_(2)^(*)与Pfirrmann分级的相关性,据相关系数r判断相关程度。结果L_(1)/L_(2)~L_(5)/S_(1)椎间盘在Pfirrmann分级与T_(2)^(*)值差异均有统计学意义(P<0.05)。分析L_(1)/L_(2)~L_(5)/S_(1)椎间盘在Pfirrmann分级与T_(2)^(*)值有相关性,L_(1)/L_(2)和L_(2)/L_(3)呈弱负相关,L_(3)/L_(4)呈强负相关,L_(4)/L_(5)和L_(5)/S_(1)呈中等程度的负相关。T_(2)^(*)值随着Ⅰ~Ⅴ组排序信号的变化呈现出下降趋势,尤其是Ⅱ~Ⅴ级的T_(2)^(*)值下降明显。结论基于MRI mDIXON-Quant序列T_(2)^(*)值定量评估腰椎间退行性变的程度不仅扫描时间短、重复性高、操作简便,尤其对下腰椎间盘退行性变的定量分析,不易受观察者主观因素的影响,可在一定程度上评估腰椎间盘退行性变。 Objective To investigate the clinical value of mDIXON-Quant quantitative T_(2)^(*) value in the evaluation of lumbar disc degeneration.Methods From March 2019 to November 2019,a total of 98 patients aged 19~78 years were recruited for routine MRI examination and mdixon-quant examination of the lumbar spine.The inclusion criteria and the image quality were satisfactory,with an average age of(42.16±17.13)years.According to Pfirrmann standard,L_(1)/L_(2)~L_(5)/S_(1) discs were graded by Pfirrmann for disc degeneration,and the T_(2)^(*) value of corresponding discs was quantitatively measured by water-fat separation(mDIXON-Quant)technique.SPSS 19.0 software was used to compare the corresponding T_(2)^(*) values of L_(1)~S_(1) discs and Pfirrmann grades in different test subjects using general linear variance t test.P<0.05 was considered to be statistically significant.Pearson rank correlation was used to analyze the correlation between T_(2)^(*) and Pfirrmann classification,and the correlation degree was determined according to the correlation coefficient r.Results The Pfirrmann grade of L_(1)/L_(2)~L_(5)/S_(1) intervertebral discs was significantly different from that of T_(2)^(*)(P<0.05).The Pfirrmann classification of L_(1)/L_(2)~L_(5)/S_(1) intervertebral discs was correlated with T_(2)^(*),L_(1)/L_(2) and L_(2)/L_(3) were weakly negative correlated,L_(3)/L_(4) were strongly negative correlated,and L_(4)/L_(5) and L_(5)/S_(1) were moderately negative correlated.T_(2)^(*) value as groupⅠ~Ⅴsorting signal change present a downward trend,especially classⅡtoⅤT_(2)^(*) value decreased significantly.Conclusion The degree of lumbar intervertebral disc degeneration was quantitatively evaluated based on the T_(2)^(*) value of mdixon-quant sequence in mri.Not only was the scanning time short,the repeatability high,and the operation simple,but the quantitative analysis of the degeneration of the lower lumbar disc was not easily affected by the subjective factors of the observer,so the degeneration of the lumbar disc
