摘要
目的探讨血清肌酐(Scr)联合尿微量白蛋白/肌酐比值(UACR)在诊断冠心病合并肾衰竭患者中的临床应用价值。方法收集2019年10月至2020年5月进行全面体检,其中体检结果都正常的健康体检者50例为健康对照组;收集同期确诊的冠心病患者50例为冠心病组;另外收集同期确诊的冠心病合并肾衰竭患者50例为冠心病合并肾衰组。对比3组研究对象血清Scr、尿肌酐UCr、尿微量白蛋白mALB、尿UACR水平。结果健康对照组的血清Scr、尿肌酐UCr、尿微量白蛋白mALB和尿UACR水平最低,而冠心病合并肾衰组的研究对象的血清Scr、尿肌酐UCr、尿微量白蛋白mALB和尿UACR水平最高,冠心病组的次之。血清Scr联合尿UACR水平检测在冠心病合并肾衰组早期诊断中具有明显的临床意义(P<0.05)。Pearson相关分析显示,血清Scr和尿UACR水平在冠心病合并肾衰竭组呈正相关(P<0.05)。结论对于冠心病合并肾衰竭的早期诊断,血清Scr联合尿UACR检测具有重要的诊断参考价值,应推广应用。
Objective To investigate the clinical significance of the detection of serum creatinine(Scr)combined with urinary microalbumin/creatinine ratio(UACR)in diagnosis of coronary heart disease(CHD)complicated by renal failure.Methods A total of 50 healthy subjects who underwent physical examination in our hospital from October 2019 to May 2020 were enrolled as control group,at the same time,and 50 patients with coronary heart disease who were treated in the hospital were enrolled as CHD group,moreover,the other 50 patients with coronary heart disease complicated by renal failure were enrolled as complication group.The serum levels of creatinine(Scr),urinary creatinine(UCr),urinary microalbumin(mALB),urinary creatinine combined with urinary microalbumin/creatinine ratio(UACR)were observed and compared among the three groups.Results The serum levels of Scr,UCr,mALB,UACR in control group were the lowest,however,which in complication group were the highest,followed by the CHD group The combined detection of Scr with UACR had obviously clinical significance in diagnosis of CHD complicated by renal failure(P<0.05).Pearson correlation analysis showed that serum levels of Scr were positively correlated with those of UACR in complication group(P<0.05).Conclusion The combined detection of Scr with UACR has important clinical significance in early diagnosis of CHD complicated by renal failure,therefor,it should be popularized in clinical practice.
