摘要
[目的]观察大黄煎剂保留灌肠辅助治疗肝性脑病的临床疗效。[方法]将60例肝性脑病患者按照随机数字表法分为两组,对照组30例予常规西医治疗,观察组30例在对照组的基础上予大黄煎剂(醋制大黄30 g,乌梅30 g)保留灌肠治疗。比较两组疗效,治疗前后血氨、内毒素、肝功能指标水平,苏醒时间及苏醒后认知功能评分变化。[结果]观察组总有效率为90.0%,对照组总有效率为66.7%,观察组疗效优于对照组(P<0.05)。治疗后,两组血氨、内毒素、肝功能指标水平均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05);观察组苏醒时间短于对照组;苏醒后认知功能评分观察组高于对照组(P<0.05)。[结论]大黄煎剂保留灌肠治疗肝性脑病可有效控制肝性脑病进展,改善认知功能。
[Objective]To observe the clinical effect of retention enema therapy with Rhubarb decoction in the treatment of hepatic encephalopathy.[Methods]Sixty patients with hepatic encephalopathy were divided into two groups by random number table method. In the control group the 30 patients were treated with conventional western medicine, and the other 30 patients in the observation group were treated by adding retention enema therapy with Rhubarb decoction(Rhubarb 30 g, Fructusmume 30 g) on basis of the control group. The curative effect, blood ammonia, endotoxin, liver function levels, recovery time and cognitive function scores after recovery were compared between the two groups before and after the treatment.[Results]The total effective rate of the observation group was90.0%, and the total effective rate of the control group was 66.7%. The curative effect of the observation group was better than that of the control group(P<0.05). After treatment, the levels of blood ammonia, endotoxin and liver function indexes in both groups were lower than those before treatment(P<0.05), and the observation group was lower than the control group(P<0.05);the recovery time of the observation group was shorter than that of the control group;the cognitive function score of the observation group was higher than that of the control group(P<0.05).[Conclusion]The retention enema therapy with Rhubarb decoction in the treatment of hepatic encephalopathy can effectively control the progress of hepatic encephalopathy and improve the life quality.
