摘要
通过分析股骨—螺钉—骨板系统的应力分流情况,为股骨干骨折内固定治疗确定合理的螺钉布局提供指导。将CT扫描获得的股骨DICOM格式影像数据导入Mimics软件中,通过三维重建得到完整股骨的几何模型;在SolidWorks软件中分别建立骨板与螺钉的三维模型,将股骨、骨板及3种不同布局的螺钉装配后导入HyperWorks软件中,模拟股骨干中部横断骨折,分析双腿站立工况下3种六螺钉固定方案中股骨—螺钉—骨板系统的应力分流情况。股骨应力在螺钉连接处较为集中,骨板应力在骨折线处较为集中。八孔骨板上固定6枚螺钉空置中间两孔时,工作长度更长,骨板应力最小,骨折处骨板应力占比为87.46%。股骨—螺钉—骨板系统的应力分流研究有助于确定合理的螺钉布局,得出的结果与临床治疗股骨干横断骨折固定方案相吻合,可为进一步研究骨折内固定的生物力学特性及临床治疗提供理论依据与指导。
By analyzing the stress shunt of the femoral-screw-plate system,provide guidance for determining a reasonable screw fixation position during the treatment of femoral shaft fractures. Import the DICOM format image data of the femur obtained by the CT scan into the Mimics software,and obtain the geometric model of the complete femur through three-dimensional reconstruction;establish the three-dimensional model of the bone plate and the screw in the SolidWorks software,and combine the femur and the bone plate with three different layouts. The screws were assembled and imported into HyperWorks software to simulate the mid-femoral shaft transverse fracture to analyze the stress shunt of the femoral-screw-plate system under the condition of standing with two legs. The stress of the femur is concentrated at the screw connection,and the stress of the bone plate is concentrated at the fracture line;When the six screws are fixed on the eight-hole bone plate and the two middle holes are empty,the working length is longer and the stress of the bone plate is the smallest. The stress ratio of the bone plate at the fracture is 87.46%. The stress shunt study of the femoral-screw-plate system is helpful to determine the reasonable screw layout,and the results obtained are consistent with the clinical treatment of femoral shaft fracture fixation. It can provide theoretical basis and guidance for further research on the biomechanical properties and clinical treatment of fracture internal fixation.
