摘要
目的评价医院-社区-家庭延续护理对脑卒中患者出院后的康复效果。方法计算机检索中国知网、万方数据库、维普、中国生物医学文献数据库、Web of Science、The Cochrane Library、PubMed、EMbase数据库,检索时限为建库至2021年3月,对照组采用医院主导的常规脑卒中延续护理,试验组采用医院-社区-家庭模式下脑卒中延续护理的随机对照试验(RCT),由2名研究人员独立进行文献筛选,质量评价和资料提取,采用Stata 16统计学软件进行meta分析。结果纳入21项RCT,共1968例患者。meta分析结果显示:与对照组比较,试验组患者日常生活能力[SMD=1.26,95%CI(0.84,1.69),P<0.01]、运动功能[SMD=1.40,95%CI(1.03,1.77),P<0.01]评分均升高,神经功能缺损[WMD=-2.14,95%CI(-2.48,-1.81),P<0.01]、焦虑[SMD=-1.33,95%CI(-1.73,-0.92),P<0.01]和抑郁[SMD=-1.12,95%CI(-1.45,-0.79),P<0.01]评分均降低。结论医院-社区-家庭延续护理对提高脑卒中出院患者日常生活能力、运动功能,减轻神经功能缺损、焦虑、抑郁状态具有积极显著的效果。
Objective To evaluate the rehabilitation effect of hospital-community-family continuing care for stroke patients after discharge.Methods CNKI,Wanfang Database,VIP,China Biomedical Literature Database,Web of Science,the Cochrane Library,PubMed,and EMbase databases were searched by computer,and the retrieval time from established to March 2021.The control group used hospital-led routine transitional care for stroke,and the experimental group used a randomized controlled trial(RCT)of transitional care for stroke in hospital-community-family mode.Literature screening,quality evaluation and data extraction were carried out by two researchers independently,and meta-analysis was performed by Stata 16 statistical software.Results A total of 21 RCTs involving 1968 patients were included.The results of meta-analysis showed that:compared with the control group,the daily living ability(SMD=1.26,95%CI[0.84,1.69],P<0.01)of patients in the experimental group were improved,and the motor function(SMD=1.40,95%CI[1.03,1.77],P<0.01),and neurological deficits(WMD=-2.14,95%CI[-2.48,-1.81],P<0.01),anxiety(SMD=-1.33,95%CI[-1.73,-0.92],P<0.01),and depression(SMD=-1.12,95%CI[-1.45,-0.79],P<0.01)were decreased.Conclusion Hospital-community-family transitional nursing has positive and significant effect on improving the daily living ability and motor function of discharged patients with stroke,and alleviating the neurological deficit,anxiety and depression.
