妊娠期糖尿病孕妇血清脂肪因子Apelin水平与糖脂代谢相关性 被引量:8

Relationship between serum adipokine Apelin level of pregnant women with gestational diabetes mellitus and their glucose and lipid metabolism

摘要 目的:分析血清脂肪因子Apelin与妊娠期糖尿病(GDM)孕妇糖脂代谢关系。方法:选取2019年1月-2020年2月本院收治的GDM孕妇60例为观察组,规律产前检查健康孕妇60例为对照组,分别于孕中期(24~28周)和孕晚期(29~32周)检测两组血清Apelin、空腹血糖(FPG)、OGTT 1h PG、OGTT 2h PG、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI)、β细胞功能指数(HOMA-β)以及总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)等。根据观察组GDM治疗情况分组,糖化血红蛋白(HbA1c)>6.5%(血糖控制不良组)和≤6.5%(血糖控制良好组),检测治疗后血清脂肪因子Apelin水平。结果:孕中期与孕晚期时观察组FPG、1h PG、2h PG FINS HOMA-IR均高于对照组,ISI、HOMA-β低于对照组,TC、TG、LDL-C均高于对照组,HDL-C低于对照组,血清Apelin水平(478.23±32.77 pg/ml、495.62±35.14 pg/ml)高于对照组(389.19±22.56 pg/ml、392.51±23.55 pg/ml)(均P<0.05);且观察组孕晚期时FPG、1h PG、2h PG FINS HOMA-IR均高于孕中期,ISI、HOMA-β低于孕中期,TC、TG、LDL-C均高于孕中期,HDL-C低于孕中期,血清Apelin水平高于孕中期(均P<0.05)。观察组治疗后,血糖控制不良组血清Apelin水平(503.61±40.17 pg/ml)高于血糖控制良好组(399.89±23.29 pg/ml)(P<0.05)。Pearson相关性显示,血清Apelin与GDM孕妇FPG、1h PG、2h PG、FINS、HOMA-IR等血糖代谢指标均呈正相关性,与ISI、HOMA-β均呈负相关,与TC、TG、LDL-C均呈正相关性,与HDL-C呈负相关性(均P<0.05)。结论:GDM孕妇血清Apelin水平升高,治疗后下降。血清Apelin升高可能与GDM糖脂代谢紊乱的发生有关。 Objective:To analyze the relationship between serum adipokine Apelin level of pregnant women with gestational diabetes mellitus and their glucose and lipid metabolism(GDM).Methods:60 pregnant women with GDM were selected in observation group and another 60 healthy pregnant women were selected in control group from January 2019 to February 2020.The levels of serum adipokine Apelin,fasting plasma glucose(FPG),1h and 2h PG of OGTT,and fasting insulin(FINS),the values of insulin resistance index,such as homeostais model assessment of insulin resistance(HOMA-IR)and insulin sensitivity index(ISI),the value of beta cell function index,such as homeostasis model assessment of beta cell(HOMA-beta),and the levels of total cholesterol(TC),triglycerides(TG),low density lipoprotein cholesterol(LDL-C),and high density lipoprotein cholesterol(HDL-C)of the women in the two groups during the second trimester of pregnancy(24-28 gestational weeks)and during the third trimester of pregnancy(29-32 gestational weeks)were detected.According to the HbA1c level after treatment,the women in the observation group were further divided into group A(women with HbA1c>6.5%)and group B(women with HbA1c≤6.5%),and the serum adipokine Apelin level of the women in the observation group after treatment was also detected.Results:The levels of FPG,1h and 2h PG,and FINS,and the HOMA-IR value of the women in the observation group during the second and the third trimester of pregnancy were significantly higher than those of the women in the control group,but the ISI level and the HOMA-βvalue of the women in the observation group were significantly lower.The levels of TC,TG,and LDL-C of the women in the observation group during the second and the third trimester of pregnancy were significantly higher than those of the women in the control group,but the HDL-C level of the women in the observation group was significantly lower.The serum adipokine Apelin level of the women in the observation group during the second or the third trimester of pregnancy(478.23
