摘要
目的探讨尿常规与C反应蛋白(CRP)检验对小儿急性阑尾炎的诊断价值。方法100例小儿急性阑尾炎患儿,均采用尿常规检验及CRP检验。比较两种检验方式对小儿急性阑尾炎的阳性检出率、检测用时及对不同类型急性阑尾炎的阳性检出率。结果CRP检验小儿急性阑尾炎的阳性检出率为98%(98/100),高于尿常规检验的79%(79/100),差异有统计学意义(P<0.05)。尿常规检验的检测用时为(1.4±0.6)h,短于CRP检验的(3.2±0.7)h,差异有统计学意义(P<0.05)。CRP检验急性单纯性阑尾炎的阳性检出率高于尿常规检验,差异有统计学意义(P<0.05);CRP检验与尿常规检验急性化脓性及坏疽性阑尾炎的阳性检出率比较,差异无统计学意义(P>0.05)。结论尿常规检验在急性阑尾炎中的阳性检出率明显低于CRP检验,但尿常规检验的检测用时短于CRP检验,临床医生可根据临床实际情况以及尿常规检验和CRP检验的特点和优势选择合适的检验项目或进行联合检验。
Objective To discuss the value of routine urine test and C-reactive protein(CRP)test in clinical diagnosis of acute appendicitis in children.Methods 100 children with acute appendicitis all underwent routine urine tests and CRP tests.The positive detection rate,testing time and the positive detection rate in different types of acute appendicitis were compared between the two tests.Results The positive rate of CRP test for acute appendicitis in children was 98%(98/100),which was higher than 79%(79/100)of routine urine test,and the difference was statistically significant(P<0.05).The detection time of routine urine test was(1.4±0.6)h,which was shorter than(3.2±0.7)h of CRP test,and the difference was statistically significant(P<0.05).The positive detection rate of CRP test for acute simple appendicitis was higher than that of routine urine test,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the positive detection rate of acute purulent and gangrenous appendicitis between CRP test and routine urine test(P>0.05).Conclusion The detection rate of routine urine test in acute appendicitis are significantly lower than that of C-reactive protein test,but the detection time of routine urine test is shorter than that of CRP test.Clinicians can select test items or combined tests according to the clinical situation and the characteristics and advantages of routine urine test and CRP test.
