摘要
目的:探究改良外剥内扎术在环状混合痔中的效果及对炎症指标的影响。方法:选取2018年6月—2020年6月在常州市金坛区人民医院肛肠科就诊的92例环状混合痔疮患者作为研究对象,按随机数表法分为对照组、观察组,每组各46例,对照组采用外剥内扎术治疗,观察组采用改良外剥内扎术治疗,观察两组患者手术时间、住院时间及术中出血量。术后并发症发生率、临床疗效以及对手术前后炎症指标水平。结果:观察组的手术时间(32.15±5.25)min高于对照组(t=4.858,P<0.05),而住院时间(7.26±2.13)d及术中出血量(13.25±2.12)mL均低于对照组,差异有统计学意义(t=6.863、12.394,P<0.05);观察组的尿潴留发生率低于对照组,差异有统计学意义(χ^(2)=0.303,P>0.05),肛门狭窄发生率低于对照组,差异有统计学意义(χ^(2)=6.405,P<0.05)。观察组术后一个月临床疗效的总有效率明显高于对照组,差异有统计学意义(t=4.039,P<0.05)。术后,观察组C反应蛋白(CRP)和白细胞介素-6(IL-6)炎症指标的水平明显低于对照组,差异有统计学意义(t=6.035、9.432,P<0.05)。结论:采用外痔切剥内痔分段的改良外剥内扎术能明显提高临床治疗效果、更好地改善患者机体的炎症状态,缩短患者症状恢复时间,临床上值得推广应用。
Objective:To explore the effect of modified external stripping and internal ligation in circular mixed hemorrhoids and its influence on inflammation indicators.Methods:A total of 92 patients with circular mixed hemorrhoids who were treated in the hospital from June 2018 to June 2020 were selected as the research objects.They were divided into control group and observation group according to a random number table,with 46 cases in each group.The control group was treated with external stripping and internal ligation,and the observation group was treated with modified external stripping and internal ligation.The operation time,hospitalization time and intraoperative blood loss,the incidence of postoperative complications,clinical efficacy and treatment inflammation index levels before and after surgery of the two groups were observed.Results:The operation time of the observation group(32.15±5.25)min was higher than that of the control group,with statistically significant difference(t=4.858,P<0.05),while the hospitalization time(7.26±2.13)d and intraoperative blood loss(13.25±2.12)mL were lower than those of the control group with statistically significant difference(t=6.863,12.394,P<0.05).The incidence of urinary retention in the observation group was lower than that in the control group,with no statistically significant difference(χ^(2)=0.303,P>0.05).But the incidence of anal stenosis in the observation group was lower than that in the control group,with statistically significant difference(χ^(2)=6.405,P<0.05)The total effective rate of clinical curative effect one month after operation in the observation group was significantly higher than that in the control group,with statistically significant difference(χ^(2)=4.039,P<0.05).After operation,the levels of C-reactive protein(CRP)and interleukin-6(IL-6)in the observation group were significantly lower than those in the control group,with statistically significant difference(t=6.035,9.432,P<0.05).Conclusion:The modified external hemorrhoids by cutting and strip
