摘要
本文收集了2例罕见厌氧菌感染所致肺脓肿患者,2例患者分别为短真杆菌和栖牙普雷沃菌感染。患者初期经验性抗感染治疗无效,常规痰培养不能明确病原学诊断,最后通过CT引导下经皮肺组织穿刺活检,结合病理学改变,并通过送检二代测序技术明确病原学诊断,进而针对病原学进行治疗,使患者病情好转。本文总结了2例肺脓肿患者的临床表现、影像、病理以及病原微生物等特点,并查阅相关文献,提高对罕见厌氧菌感染导致肺脓肿的认识,总结临床经验。同时也证明了经皮肺组织穿刺活检联合二代测序技术对肺脓肿病原学诊断上的价值和意义,值得在临床上进一步推广。
This paper includes two patients with pulmonary abscess who were respectively infected with rare anaerobes,namely Eubacterium brachy and Prevotella denticola.The initial empirical anti-infective treatment was ineffective,and the pathogenic diagnosis could not be clarified by routine sputum culture.Fortunately,the pathogenic diagnosis was clarified by CT-guided percutaneous puncture biopsy of pulmonary tissues,combined with pathological changes and by sending pulmonary tissues for inspection by next generation sequencing technology,which further guided the treatment of etiology and then promoted the improvement of the patients’conditions.This paper intends to improve the understanding of rare anaerobes causing pulmonary abscess and summarize clinical experience through summarizing the clinical manifestations,imaging,pathology,pathogenic microorganisms and other characteristics of two patients with pulmonary abscess,and reviewing the relevant literature.It also demonstrates the value and significance of percutaneous puncture biopsy of pulmonary tissues combined with next generation sequencing technology on the pathogenic diagnosis of pulmonary abscess,which is worthy of further promotion in clinical practice.
