摘要
目的探索双绒双羊双胎妊娠孕妇孕中期体重增长对围产期母儿结局的影响。方法前瞻性招募符合研究标准的早孕期双绒双羊双胎孕妇,收集基线信息、孕中期体重增长数据和围产期母儿结局资料。按2009年美国医学研究所指南标准,将受试者按孕中期体重增长速率分为低于指南推荐组、符合指南推荐组和高于指南推荐组进行差异性分析,并采用线性回归和逻辑回归进行相关性分析。结果共有177例双绒双羊双胎孕妇纳入研究分析。孕中期体重增长速率低于指南推荐组平均胎儿出生体重最低,高于指南推荐组平均胎儿出生体重最高(P<0.05)。进一步分析发现,孕中期前期体重增长速率与平均胎儿出生体重呈正相关(β=44.48,95%CI 24.21~64.74),与至少1个为小于胎龄儿风险呈负相关(OR=0.76,95%CI 0.59~0.97)。结论双绒双羊双胎妊娠孕妇孕中期体重增长与胎儿出生体重相关。
Objective To explore the effect of mid-pregnancy weight gain on perinatal maternal and child outcomes in pregnant women with twin chorion and twin lamb pregnancies.Methods Pregnant women with twin chorion and twin lamb pregnancies in early pregnancy who met the study criteria were prospectively recruited.Data of baseline information,mid-pregnancy weight gain and perinatal maternal outcome were collected.According to the 2009 Institute of Medicine guideline criteria,Subjects were divided into three groups according to the weight gain rate in mid-pregnancy:lower than the guideline recommendation,in line with the guideline recommendation and above the guideline recommendation.Linear regression and logistic regression were used for correlation analysis.Results A total of 177 pregnant women with twin chorion and twin lamb pregnancies were included.The mean fetal birth weight was lowest with the weight gain rate in mid-pregnancy in the below-guideline recommended group and highest in the above guideline recommended group(P<0.05).Further analysis revealed that the rate of weight gain in early mid-pregnancy was positively associated with mean fetal birth weight(β=44.48,95%CI 24.21-64.74)and negatively associated with the risk of having at least one small for gestational age(OR=0.76,95%CI 0.59-0.97).Conclusion Mid-pregnancy weight gain in pregnant women with twin chorion and twin lamb pregnancies is associated with fetal birth weight.
